锁定加压接骨板内固定治疗肱骨近端骨折

目的探讨肱骨近端锁定接骨板内固定治疗中老年肱骨近端粉碎性骨折的临床疗效。方法自2005年5—2007年1月，应用肱骨近端锁定接骨板内固定治疗肱骨近端粉碎性骨折12例，其中男4例，女8例；左侧5例，右侧7例；年龄60-75岁，平均66岁。根据Neer分类：二部分骨折7例，三部分骨折4例，四部分骨折1例，均为闭合性新鲜骨质疏松性骨折。采用三角肌、胸大肌间隙入路，9例行自体骨植骨术。结果12例均得到随访，随访时间4-18个月，平均10个月。所有骨折均愈合，平均愈合时间12周，根据Constant评分，优5例，良5例，中2例，优良率83．3％。结论肱骨近端锁定接骨板内固定治疗中老年肱骨近端粉碎性骨折具有固定可靠、退钉率低、并发症少等优点；必要时植骨，可提高骨折愈合率，特别适用于骨质疏松的肱骨近端粉碎性骨折。

Locking proximal humerus plate for wrinkly humeral proximal comminuted fractures

Objective To exlore the clinical effect of locking proximal humerus plate（LPHP） for the wrinkly humeral proximal comminuted fractuers.Methods From May 2005 to January 2007, 12 cases were applied LPHP for the treatment of humeral proximal comminuter fractures, of 92l the patients, 4 were males and 8 females; 5 left and 7 right; aging from 60-75, average age was 63 years. According to Neer classification, 7 cases were two part fractures, 4 three part fractures and 1 cases four part fractures, all the cases were closed freshly fragile fracture. By using the gap between deltoid and ectopectoralis, 9 cases autogolous bone graft.Results 12 patients were followed-up, and the period ranged from 4 to 18 months, the average time was 10 months. All fractures healed for average of 12 months, according to the Constant scoring, excellent clinical results achieved in 5 cases, good in 5 fair in 2 with the excellent and good rale was 88.8%.Conclusion the method of LPHP is a good operation to treat the humeral proximal osteoporosis comminuted fractures.
