家庭肠内营养患者营养支持效果研究

目的 通过对实施家庭肠内营养患者营养支持效果的研究,探讨适合我国国情的家庭肠内营养支持模式.方法 回顾性分析北京军区总医院营养科24例实施家庭肠内营养患者的营养状况,观察比较患者肱三头肌皮褶厚度、上臂肌围、白细胞、红细胞、血红蛋白、血糖、总胆固醇、三酰甘油、谷丙转氨酶、谷草转氨酶、总胆红素、直接胆红素、乳酸脱氢酶、碱性磷酸酶、胆碱酯酶、尿素氮、肌酐、尿酸、钾离子、钙离子、铁离子等指标改善的情况.结果 营养支持后白蛋白(34.3 ±2.8) g/L]、前白蛋白(182 ±66)mg/L]与营养支持前(31.8±4.8) g/L、(147±55) mg/L]比较,差异均有统计学意义(均P＜0.05).营养支持后肱三头肌皮褶厚度、上臂肌围、白细胞、红细胞、血红蛋白、血糖、总蛋白、总胆固醇、三酰甘油、谷丙转氨酶、谷草转氨酶、总胆红素、直接胆红素、乳酸脱氢酶、碱性磷酸酶、胆碱酯酶、尿素氮、肌酐、尿酸、钾离子、钙离子、铁离子与营养支持前比较,差异均无统计学意义(P＞0.05).结论 合理的家庭营养支持方案,配合有效的营养指导,对维持家庭肠内营养患者营养状况和改善营养不良具有重要意义.

Effect of nutritional support of 24 patients with home enteral nutrition

Objective To explore a practical home enteral nutritional support in Chinese patients. Methods Nutritional data of twenty-four patients before and after home enteral nutrition support were analyzed ret- rospectively, triceps skinfold thickness, arm muscle circumference, white blood cell, red blood cell, blood sugar, hemoglobin, total cholesterol, triglyeeride, glutamie-pyruvic transaminase, glutamie oxalaeetic transaminase, total bilirubin, direct bilirubin, lactic dehydrogenase, alkaline phosphatase, cholinesterase, urea nitrogen, ereatinine, uric acid, potassium ions, calcium ions, iron ions of patients were observed and compared. Results There were significant differences on the albumin （34.3 ＋2.8）g/L] and prealbumin （ 182 ＋66）mg/L] after nutrition com- pared with before support （ 31.8 ＋ 4.8 ） g/L, （ 147 ＋ 55 ） mg/L 3 （ P 〈 0.05 ）. There were no significant differences on triceps skinfold thickness, ann muscle circumference, white blood cell, red blood cell, hemoglobin, blood sug- ar, total protein, total cholesterol, triglyceride, glutamic-pyruvie transaminase, glutamie oxalacetic transaminase, total bilirubin, direct bilirubin, lactic dehydrogenase, alkaline phosphatase, eholinesterase, urea nitrogen, ereati- nine, uric acid, potassium ions, calcium ions, iron ions of before support compared with after nutrition （P 〉 0.05 ）. Conclusion Practical nutritional support and effective nutritional guidance are crucial to the maintenance of nutritional status and improvement of malnutrition.
