奥沙利铂联合亚叶酸钙和氟尿嘧啶新辅助化疗治疗进展期胃癌临床疗效观察

目的:探讨奥沙利铂联合亚叶酸钙和氟尿嘧啶方案新辅助化疗治疗进展期胃癌的临床疗效。方法:将62例进展期胃癌患者随机分为两组,对照组30例采用单纯手术治疗,观察组32例行奥沙利铂联合亚叶酸钙和氟尿嘧啶方案新辅助化疗后再行手术治疗,观察新辅助化疗后临床有效率及毒副反应,比较两组无肿瘤残留(R0)切除率。结果:观察组化疗有效率为62.50%。化疗的毒副反应主要是消化道反应、骨髓抑制及神经系统毒性等,且多为轻度,能耐受。观察组R0切除率为71.88%,对照组R0切除率为53.12%,观察组明显高于对照组,差异有统计学意义(P<0.05)。结论:奥沙利铂联合亚叶酸钙和氟尿嘧啶方案新辅助化疗治疗进展期胃癌疗效好,毒副反应轻,大大提高了进展期胃癌的R0切除率,值得广泛应用。

The Clinical Efficacy of Oxaliplatin Combined with Calcium Folinate and Fluorouracil in the Treatment of Advanced Gastric Cancer

Objective：To investigate the clinical effect of oxaliplatin combined with calcium folinate and fluorouracil in the treatment of advanced gastric cancer.Methods：62 cases of advanced gastric cancer patients were randomly divided into two groups,the control group of 30 cases was treated with operation treatment,and the observation group of 32 cases was treated with operation treatment of neo-adjuvant chemotherapy of oxaliplatin combined with fluorouracil and calcium folinate scheme.The clinical efficiency rate and toxicity reaction after neoadjuvant chemotherapy were taken for observation,and the residual tumor resection rate（R0） of the two groups were compared.Results：The effective rate of chemotherapy in the observation group was 62.50%.The toxicity of chemotherapy was mainly gastrointestinal reaction,bone marrow suppression and nervous system toxicity,and mostly mild,tolerance.The R0 resection rate in the observation group was 71.88%,R0 resection rate in the control group was 53.12%.The observation group was significantly higher than the control group,the difference was statistically significant（P0.05）.Conclusion：The therapeutic effects of neoadjuvant chemotherapy of oxaliplatin combined with calcium folinate and fluorouracil for advanced gastric cancer is significant,and the side effect is mild.It greatly improves the gastric cancer R0 resection rate,and is worthy of widely application.
