精索静脉曲张不育症患者精索静脉血气分析

目的探讨精索静脉曲张(VC)导致男性不育的发病机制。方法选择50例VC不育症患者及40例对照组作为研究对象,分别取其左侧精索内静脉以及周围静脉血进行血气分析,并进行比较。结果 (1)VC不育症患者左侧精索静脉血中的pH、PO2、SO2及HCO-3低于外周静脉血(P<0.05),PCO2及K+高于外周静脉血(P<0.05),而其他指标差异无统计学意义(P>0.05)。(2)VC不育症患者左侧精索静脉血中的pH、PO2、SO2及HCO-3低于对照组(P<0.05),PCO2及K+高于对照组(P<0.05),而其他指标差异无统计学意义(P>0.05)。(3)VC不育症患者精液密度及活率低于对照组(P<0.05),畸形率高于对照组(P<0.05)。结论 VC不育症患者精索静脉中的pH、PO2、SO2、HCO-3降低和PCO2、K+升高是导致精液密度及活率降低同时畸形率升高进而引起男性不育的原因。

Analysis of Spermatic Vein Blood Gas from the Infertility patients with varicocele

Objective To investigate the nosogenesis why varicocele may result in infertility.Methods The left spermatic and peripheral vein blood was taken from 50 infertility patients with varicocele and from 40 normal volunteers,and their blood gas was measured.Results(1) The pH,PO2,SO2 and HCO3 of the left spermatic vein blood were lower than that of the peripheral vein blood in the males with varicocele(P < 0.05),the PCO2 and K+ of the left spermatic vein blood were higher than that of the peripheral vein blood in the male with varicocele(P < 0.05),and the other indexes were not significantly different between the two groups of vein blood(P > 0.05).(2) The pH,PO2,SO2 and HCO3 of the left spermatic vein blood from the males with varicocele were lower than that from the normal males(P < 0.05),the PCO2 and K+ of the left spermatic vein blood from the males with varicocele were higher than that from the normal male(P < 0.05),but the other indexes were not significantly different(P > 0.05).(3) The density and the motility of sperm from the males with varicocele were both lower than that from the normal males(P <0.05),and the abnormal rate of sperm from the males with varicocele was higher than that from the normal males(P < 0.05).Conclusion The depression of pH,PO2,SO2 and HCO3 and the rise of the PCO2 and K+ in spermatic vein blood from infertility patients with varicocele may be the causes of the depression of density and motility of sperm and of the rise of the abnormal rate of sperm which can result in infertility.
