46例转移性肺癌介入治疗疗效观察

目的:探讨支气管动脉灌注化疗栓塞(BACE)与双灌注治疗转移性肺癌的临床疗效。方法:采用改良式Seldinger技术,行股动脉或股静脉穿刺,选择性支气管动脉或肺动脉插管,对38例转移性肺癌行支气管动脉灌注化疗,部分患者加用明胶海绵或碘油栓塞,8例行支气管动脉和肺动脉双灌注。结果:肿瘤消失、症状完全缓解7例,肿瘤缩小以及胸痛、咳嗽等症状改善26例(其中肺不张复张2例),肿瘤无改变6例,肿瘤增大或数目增多、症状加重7例,介入治疗后1年、2年生存率分为56.5%(26/46)和19.5%(9/46)。结论:支气管动脉灌注化疗栓塞与双灌注是治疗转移性肺癌的有效方法之一。可以确切延长中位生存期,提高患者生存质量。

Observation of therapeutic effect of interventional therapy in the treatment of pulmonary metastasis

Objective： To study the therapeutic effect of Transcatheter Bronchial Artery Chemoembolization（TBACE） and double arterial infusion in the treatment of pulmonary metastasis.Methods：Using modified Seldinger technique and super selective catheterization,thirty-eight pulmonary metastasis patients were infused anti-neo-plastica agents via the catheter,and embolized the bronchial artery at last;Eight patients were treated with double arterial infusion.Results： the tumor disappeared and the symptoms alleviated completely in 7 cases.Tumor re-duced in size or number and the symptoms of chest pain,spirit nasty,cough etc.improved in 26 cases（Among them,the lung was flaring again in 2 cases）.Tumor didn＇t change apparently in 6 cases.The tumor enlarged and the symptoms aggravated in 7 cases.The survival rates of this kind treatment were 56.5% and 19.5%,respective-ly,after 1 year and 2 year treatment.Conclusion： Interventional therapy is one of the most effective therapeutic methods for pulmonary metastasis treatment.It can prolong the patients＇ life and improve their quality of life.
