摘    要:目的观察双水平无创正压(BiPAP)通气对煤工尘肺肺心病患者心肺功能异常时治疗效果。方法选择64例煤工尘肺肺心病患者,均伴有不同程度的心力衰竭和肺功能低下,随机分为治疗组和对照组,两组常规治疗相同,治疗组再加Bi-PAP呼吸机双水平气道正压通气。每天治疗2次,每次持续2~4小时,治疗6周。观察两组治疗前后心肺功能指标的变化。结果治疗组通气治疗后PaO2FEV1/FVC6min步行实验均明显优于对照组,治疗组的心肺功能明显改善。结论 BiPAP通气能明显改善煤工尘肺肺心病患者的心肺功能,提高尘肺肺心病患者的生活质量。

Abstract:Objective To observe the effect of noninvasive bilevel positive airway pressure(BiPAP)ventilation treatment to the corpulmonale patients with coal worker's pneumoconiosis when they've got abnormal cardiopulmonary function.Methods 64 cases of coal workers who had pneumoconiosis and corpulmonale with different degrees of heart failure and low pulmonary function were chosen,and they were divided into therapeutics group and control group randomly.Both groups were treated with the same ordinary method.But the therapeutics group added BiPAP.The treatment was twice a day,2~4 hours per time with a total 6 weeks long.The change of pre and post therapeutic cardiopulmonary function in these 2 groups was observed.Results After the ventilation treatment,PaO2 FEV1/FVC 6min test of the therapeutics group were obviously prior to the control group and the cardiopulmonary function of the therapeutics group had distinct improvement.Conclusion BiPAP ventilation can evidently improve the miner-patients' pneumoconiosis and corpulmonale cardio-pulmonary function and their living quality as well.
