急诊胆囊切除时伴特殊合并症的手术处理

目的探讨在急诊胆囊切除术时伴特殊合并症的手术处理方法及其临床应用价值。方法对72例急诊胆囊切除术时伴特殊合并症的手术结果及随访6个月-8年的资料进行分析。结果 72例患者无一例发生手术死亡,在围手术期内亦无胆漏、胆道梗阻等并发症发生,短期效果满意;在6个月-8年的追踪随访中,1例反复并发胆源性胰腺炎经十二指肠乳头切开成型术治愈。结论对急诊胆囊切除术时伴特殊合并症患者,在手术中根据病情采用个体化手术治疗方法是可行、有效的。

Operative management on concomitant special complications during acute cholecystectomy

Objective To investigate the operative management and clinical value on concomitant special complications during acute cholecystectomy.Methods 72 patients accompanying with special complications received the emergency cholecystectomy were included in this study.The operative effective was followed-up for 6-month ～ 8-year.Results There was no patients expired after operation,no complications such as bile leakage and obstruction of biliary tract occurred in the perioperative period.1 patient accompanying with repeated biliary pancreatitis received duodenal papilla sphnicterotomy and sphnicteroplasty and fully recovered.Conclusions Different operative methods according to the special complications during the acute cholecystectomy were feasible and effective.
