应变率显像对糖尿病患者左室心肌局部收缩及舒张功能的研究

摘    要:目的应用应变率显像评价糖尿病患者左心室局部心肌纵向收缩和舒张功能。方法分别获取22例糖尿病患者(DM)和21例正常对照者标准心尖位左心长轴观、二腔观和四腔观图像,分析左室各室壁长轴方向基底部、中部心肌应变率曲线,计算各节段应变率及左室中部、基底部水平的舒张早期和心房收缩期的平均应变率。测量左室射血分数(EF%)和二尖瓣口血流的E、A峰值,计算E/A比值。结果与正常对照组比较,DM组左室各节段心肌收缩期应变率和舒张早期应变率均明显降低(P〈0.05),左室大部分节段心房收缩期应变率差异无显著性意义(P〉0.05)。在两组舒张早期和心房收缩期的平均应变率比较中差异具有显著性意义(P〈0.05)。两组间EF%、E、A及E/A差异无显著性意义。结论应变率显像为定量评价糖尿病患者局部心肌舒缩功能提供了一种新方法。

Abstract:Objective To assess the left ventricular regional systolic and diastolic function in patients with diabetes mellitus(DM)by myocardial strain rate imaging(SRI).Methods Doppler echocardiography and SRI were performed in DM(n=22)and healthy subjects(n=21)at apical long axis,two-chamber and four-chamber views.Regional systolic strain rate(SRs)and regional diastolic(SRd) were measured at middle and basal ventricule.Mean regional early diastolic strain rate(mSRe)and Mean regional later diastolic strain rate(mSRa)at the two levels were calculated.Ejection fraction and transmitral flow velocities(E,A)were also measured and E/A ratio were calculated.Results The parameters of SRs,SRe in the DM group were significantly lower than those in the healthy subjects(P<0.05-0.0001);but compared with healthy subjects,majority of SRa in DM were not different except the basal posterior wall,lateral wall and ventricular septum(P>0.05).The mSRe and mSRa of the two levels between the two groups were significantly different(P<0.05-0.0001).Conclusions SRI would be a new and practical method to quanlify the local myocardial motion.
