经椎板间隙入路显微内窥镜治疗腰椎间盘突出症31例

 目的探讨显微内窥镜腰椎间盘切除术治疗腰间盘突出临床应用并报告31例一年的随访结果.方法采用经椎板间隙入路显微内窥镜治疗腰椎间盘切除术(Micro Endscopic Discectomy,MED)技术进行髓核摘除及侧隐窝扩大术.中通过"C"型臂X线机定位、引导,将手术通道管置于椎板间隙处,电视监视下咬除部分椎板和黄韧带,摘除髓核,扩大神经根管.结果31例均获成功.术后一年随访,按改良Ma cN ab标准评定:优19例,良11例,可1例,差0例.优良率96.7%,.结论MED创伤小、疼痛轻、住院时间短、恢复快.它适应于单节段腰椎间盘突出或和同节段侧隐窝狭窄.

Translaminar microendoscopic discectomy for treating lumbosacral intervertebral disc herniation

Objective To study the clinical application of microendoscopy to the treatment of lumbosacral intervertebral disc herniation (LDH).Methods Microendoscopic surgery was used in 31 patients with LDH to excise the nucleus and decompress the lateral recess stenosis.Intraoperatively,under the localization and guidance of c-arm X-ray machine,a tube was placed between the inferior and superior lamina and the operative maneuvers were controlled by color video monitor.Results According to a modified MacNab outcome grading after one-year follow-up.results were good or excellent in 30 patients,and fair or poor in 1.ConclusionS Microendoscopic surgery is a microinvasive technique which has the advantages of minor trauma and hemorrhage,slight pain and rapid recovery.It is indicatied to single segment LDH or with lateral recess stenosis at the same level
