自体骨髓间充质干细胞移植治疗脑出血

摘    要:目的探讨自体骨髓间充质干细胞(BMSCs)移植治疗对脑出血患者外周血中内皮祖细胞活化度及神经功能的影响。方法选择2014-06—2016-06南阳南石医院86例脑出血患者,依据治疗方案不同分为2组各43例。对照组仅采取常规微创血肿清除术治疗,观察组在对照组基础上联合自体BMSCs移植治疗,统计对比2组治疗后1周、2周、3周、4周外周血内皮祖细胞活化度与治疗后3周、4周神经功能缺损评分、日常生活能力评分及神经营养因子神经生长因子(NGF)、脑源性神经生长因子(BDNF)]水平。结果观察组治疗后1周、2周、3周、4周内皮祖细胞活化度较对照组提高,差异具有统计学意义(P0.05);观察组治疗后3周、4周神经功能缺损评分较对照组降低,日常生活能力评分均较对照组提高,差异具有统计学意义(P0.05);观察组治疗后3周、4周NGF、BDNF水平均较对照组提高,差异具有统计学意义(P0.05)。结论对脑出血患者应用自体骨髓间充质干细胞移植治疗效果显著,可明显减轻患者神经功能损伤,提高内皮祖细胞与神经营养因子水平,促进其神经功能恢复,提升其日常生活能力。

Abstract:Objective To investigate the effect of autologous bone marrow mesenchymal stem cells (BMSCs) transplantation on the activation of EPCs and Neurological function in peripheral blood of patients with cerebral hemorrhage.Methods Totally 86 patients with cerebral hemorrhage were admitted to Nanshi Hospital from June 2014 to June 2016.They were divided into groups according to different treatment schemes,each with 43 cases.The control group received routine minimally invasive hematoma removal treatment,while the observation group was treated with autologous BMSCs transplantation on the basis of the control group.The activity of EPCs in peripheral blood between the two groups after 1 week,2 weeks,3 weeks and 4 weeks was compared with the neurological deficit scores,daily living ability scores and neurotrophic factorsnerve growth factor (NGF) and brain-derived neurotrophic factor (BDNF)]levels at 3 and 4 weeks after treatment.Results The activation rate of EPCs in the observation group after 1,2,3 and 4 weeks was higher than that of the control group.The difference was statistically significant (P<0.05).After 3 weeks and 4 weeks,the scores of neurological impairment in the observation group were lower than that in the control group,and the scores of daily living ability were improved compared with the control group,the difference was statistically significant (P<0.05).The levels of NGF and BDNF in the observation group after 3 weeks and 4 weeks were higher than those in the control group,the difference was statistically significant (P<0.05).Conclusion Autologous bone marrow mesenchymal stem cells transplantation is effective in the treatment of intracerebral hemorrhage.It can obviously alleviate the damage of nerve function,raise the level of EPCs and neurotrophic factors,promote the recovery of nerve function and enhance their daily living ability.
