大鼠颈内动脉线栓法制备局灶性脑缺血模型影响因素探讨

摘    要:背景:实验性脑缺血动物模型对研究脑卒中发病机制、治疗及康复评价具有重要意义。影响模型成功及稳定性因素为提高模型成功率提供实验依据。目的:探讨影响大鼠大脑中动脉局灶性脑缺血模型成功率的相关因素。设计:随机对照实验研究。地点和对象:实验地点:解放军济南军区总医院全军神经内科中心动物实验室。动物:雄性Wistar大鼠75只,体质量260~350g,共分5组(n=15):A组:颈外动脉结扎(externalcarotidarteryocclusion,ECAO) 翼腭动脉结扎(pterygopalarteryocclusion,PPAO) 聚乙烯醇栓线;B组:E-CAO PPAO 硅胶栓线;C组:ECAO 聚乙烯醇栓线;D组:ECAO 硅胶栓线;E组:假手术组。干预:A和B组采用丝线结扎翼腭动脉,近心端结扎颈总动脉,血管夹夹闭颈总动脉远端,在颈总动脉上剪一小口,边松血管夹,边进线(A组用聚乙烯醇栓线,B组用硅胶栓线),至有阻力不能再进为止。C和D组不结扎翼腭动脉,其余步骤相同。E组手术和结扎动脉与缺血动物相同,但颈内动脉内不栓线。主要观察指标:动物模型出现率;梗死体积。结果:A,B组动物的模型出现率较C,D组高,模型成功动物栓线进入颅内长度多在7mm左右,而不成功动物则多在4mm以下。A,C组较B,D组神经缺损症状出现早,同一时间内的梗死体积显著增大。结论:翼腭动脉结扎与否是影响模型成功率高低

Abstract:BACKGROUND: The experimental cerebral ischemic animal model has important significance in the investigation of the pathogenesis, therapy and rehabilitative evaluation of cerebral apoplexy. Factors that influence the success and stability of the model provide an experimental gist in the improvement of the successful rate in model establishment.OBJECTIVE: To investigate the correlative factors that affect the establishment and successful rate of the focal cerebral ischemic model in middle cerebral artery in rats.DESIGN: A randomised controlled trial study.SETTINGS and MATERIALS: Setting: central animal laboratory of the Department of Neurology of General Hospital of Jinan Command PLA. Animals: Seven-five male Wistar rats with a body mass from 260 g to 350 g were obtained. Rats were randomly allocated into 5 groups( n = 15):group A: external carotid artery occlusion(ECAO) + pterygopal artery occlusion(PPAO) + polyviol thread; group B: ECAO + PPAO + silica gel thread;group C: ECAO + polyviol thread; group D: ECAO + silica gel thread; group E: sham-operation group.INTERVENTION: PPA was ligated with silk thread in rats of group A and B as well as common carotid artery at the cardiac end. The distal end of common carotid artery was closed by vascular clamp. A small incision was made on common carotid artery. The thread was inserted with the relaxation of vascular clamp(polyviol thread was used in group A and silica gel thread was used in group B) until the thread was stopped by resistance. PPA was not ligated in groups C and D but the same rest steps as above. Operation and arterial ligation were same as ischemic groups in group E but no thread insertion in internal carotid artery.MAIN OUTCOME MEASURES: Incidence of animal model and infarct volume.RESULTS: Incidence of animal model was higher in rats of group A and B than that of group C and D. The length of the thread inserted into cranium in successful model was about 7 mm but below 4 mm in unsuccessful animal.Neural defect symptoms occurred earlier in group A and C than those of group B and D with significantly bigger infarct volume in the same period.CONCLUSION: Whether PPA is ligated or not is an important factor that affects the successful rate of the animal model and the depth of the thread inserted into middle cerebral artery is the key to the success of animal model establishment.
