经尿道气化联合电切术治疗前列腺增生症

摘    要:目的:探讨前列腺增生症(BPH)的有效治疗方法。方法:采用经尿道前列腺汽化术(TUVP)联合经尿道前列腺电切术(TURP)治疗BPH86例。结果:临床效果满意,术后6个月随访,国际前列腺症状评分平均由29.4%降为8.2分,最大尿流平均由6.4ml/s升至17.8ml/s,生活质量改善4分;残余尿量由75-500ml(19例)降至10-35ml(16例)。无严重并发症。结论:汽化和电切联合行经尿道前列腺切除术治疗BPH是两者优点的结合,临床效果好,并发症少,安全系数大,是治疗BPH的理想方法。

Abstract:Objective: The aim of this study was to undertake an evaluation of the efficacy of the combined transurethral vaporesection (TUVRP) and standard transurethral resection(TURP).Methods:In this study,86 patients with benign prostatic hyperplasia(BPH) aged 61-87(mean 72)yeas were undergone combined TUVRP and standard TURP. Patients were followed for 6 months with the evaluation of flow rates,post void residual urine volume,symptom scores(IPSS) and Quality of Life (QOL) score.Results:The total IPSS score, QOL score average and maximum flow rates improved significantly at 6 months postperatively. The quality of life has improved by 4 symptomatic units; the IPSS score decreased from 29.4 to 8.2 score and the Qmax was upgraded from 6.4ml/s to 17.8 ml/s;the residual urine decreased from 75-500ml(19men) to 10-35ml(16 men).Conclusions:Combined TUVRP and TURP prostatectomy was found to be effective for the treatment of bladder outlet obstruction due to BPE. In the assessment of the present procedure, we have registered a considerable improvement in the quality of life (QOL),an increase in the Qmax,and decrease in the IPSS score.
