高血糖对肝癌患者外科治疗后复发影响的研究

目的研究高血糖对肝癌患者外科治疗后复发的影响。方法回顾性分析2012年4月到2014年5月期间进行肝癌外科手术治疗的患者86例,其中40例高血糖(≥6.1 mol/L)患者作为高血糖组,46例血糖正常患者作为正常组,采用SPSS 19.0软件进行统计学的处理。24个月无瘤生存率采用卡方检验;空腹血糖用x珋±s表示,t检验。对参与本次研究的相关因素进行Kaplan-Meier的比例风险检验,多因素分析进行Log-rank分析,P0.05差异具有统计学意义。结果对86例患者24个月无瘤生存率进行单因素的分析,结果表明患者术中输血量200 ml,空腹血糖≥6.1 mmol/L,术后AFP8.1μg/L能够大幅度的提高患者复发率,多因素结果显示,这3个指标是影响肝癌术后无瘤生存率的相关独立危险因素,差异均有统计学意义(P0.05)。结论高血糖与肝癌患者外科治疗后复发有着密切的关联,高血糖可以提高肝癌患者复发率,临床上对患者血糖进行监控有利于预防患者肝癌患者复发,具有积极意义。

Clinical investigation of hyperglycemia and postoperative recurrence of hepatocellular carcinoma

Objective To investigate the impact of hyperglycemia on the postoperative recurrence of hepatocellular carcinoma . Method From April 2012 to May 2014, clinical data of 86 patients underwent hepatectomy were analyzed retrospectively , including 40 cases of hyperglycemia (≥6.1 mol/L) in high blood glucose group and 46 cases of normal blood glucose in normal group . Statistical analysis were performed by using SPSS 19.0 software.24-Months disease free survival was examined by chi square test . Fasting blood-glucose were presented as x±s and examined by using t test.Related risk factors were examined by using Kaplan Meier proportional hazards test and multi-factor analysis were performed by using log-rank analysis .A P value of <0.05 was considered as significant difference . Results Single factor analysis of 24-Months disease free survival was performed in 86 patients, which showed that intraoperative blood transfusion >200 ml, fasting blood-glucose ≥6.1 mmol/L and postoperative AFP>8.1μg/L might significantly increase postoperative recurrence . Multivariate results showed that above three indicators independent risk factor of are impact-free survival after hepatectomy , with significant difference (P<0.05). Conclusion There is a close correlation of hyperglycemia and postoperative recurrence of hepatocellular carcinoma.Monitoring and controlling of blood glucose of patients might help to prevent the recurrence of hepatocellular carcinoma , which is of positive clinical significance .
