老年患者嵌顿性腹股沟疝急诊手术预后影响因素的分析

目的探讨影响老年患者嵌顿性腹股沟疝急诊手术预后的因素。方法选取2014年2月至2015年2月80例老年嵌顿性腹股沟疝行急诊手术患者作为研究对象,分析患者的临床资料数据,对研究结果收集后进行统计学分析比较,观察患者肠切除和患者死亡率的相关影响因素。采用SPSS18.00软件进行统计学分析,年龄、嵌顿类型、慢性病、既往腹部手术史、术前嵌顿持续时间采用logistic回归进行数据分析。以P0.05表示差异具有统计学意义。结果既往腹部手术史(OR=1.692,P=0.001)、术前嵌顿持续时间大于24 h(OR=1.550,P=0.000)以及慢性疾病(OR=1.562,P=0.000)是肠切除手术的危险因素。患者的生存率与年龄(P=0.085)没有相关性,接受过肠切除手术和嵌顿持续时间超过24 h是影响患者生存的显著危险因素(P=0.001,P=0.000)。结论术前嵌顿疝持续时长超过24 h、腹部手术史及慢性疾病与肠切除相关。嵌顿持续时间、肠切除手术是影响老年患者嵌顿性腹股沟疝术后生存状况的重要影响因素。

Factors affecting the prognosis of elderly patients with inguinal hernia after emergency operation

Objective To investigate the factors influencing the prognosis of patients with incarcerated inguinal hernia treated by emergency surgery . Methods Eighty elderly patients with inguinal hernia who underwent emergency surgery from February 2014 to February 2015 were enrolled in this study . The clinical data of the patients were analyzed to find factors which were related to bowel resection and mortality.The SPSS18.00 software was used for statistical analysis .Logistic regression was used for the analysis of age, incarcerated type, chronic diseases, previous abdominal surgery , and preoperative duration of incarceration.P<0.05 was considered statistically significant . Results Previous abdominal surgery (OR=1.692, P =0.001), preoperative duration more than 24 hours ( OR =1.550, P =0.000) and chronic disease (OR=1.562, P=0.000) were risk factors for bowel resection .Survival was not associated with age (P=0.085).Previous intestinal surgery and preoperative duration more than 24 hours were risk factors affecting survival (P =0.001, P=0.000). Conclusions Preoperative duration more than 24 hours, incarcerated hernia, abdominal surgery history , enterectomy and chronic diseases were associated with bowel resection .Incarcerated duration and enterectomy were the important factors influencing the prognosis of patients with incarcerated inguinal hernia .
