钳式Ligasure和FOCUS超声刀在甲状腺癌手术中的应用比较

目的比较钳式Ligasure和FOCUS超声刀在甲状腺癌手术中的临床应用效果。方法回顾性分析2015年11月至2016年4月期间行甲状腺癌手术患者的临床资料,76例患者行全甲状腺切除+中央区淋巴结清扫术,由同一甲状腺专科医师完成,使用Ligasure完成手术者35例(Ligasure组),使用Focus超声刀完成手术者41例(超声刀组)。采用SPSS 19.0统计软件包分析,两组患者的肿瘤大小、术前血清钙值、PTH值、手术时间、术后24 h引流量等计量资料用(x珋±s)表示,采用t检验;ASA分级、术后并发症比较采用χ2检验,P0.05为差异有统计学意义。结果两组在手术时间、术后引流量及术后并发症的比较上差异均无统计学意义(P0.05)。两组患者均未出现永久性喉返神经损伤和永久性甲状旁腺功能减退,均未发生术后出血和切口内血肿。结论钳式Ligasure和FOCUS超声刀均能安全,有效地运用于甲状腺癌手术,达到相同的手术效果,术者可根据自身习惯选择合适的手术器械。

Clinical Application of Ligasure and Harmonic Focus in Thyroid Carcinoma Surgery

Objective To compare the clinical outcome of total thyroidectomy by using the Ligasure and Harmonic Focus. Method The clinical data of 76 patients, who underwent total thyroidectomy and prophylactic central lymph node dissection by one surgeon from November 2015 to Aprial 2016, were analyzed retrospectively , including 35 cases using Ligasure and 41 cases using Harmonic Focus .Statistical analysis were performed by using SPSS 19.0 software.Measurement data were presented as ( x±s) and examined by using t test, including tumor size , preoperative serum calcium , PTH, operation time and postoperative amount of drainage within 24 hours.ASA grading and complications were compared between two groups by using Chi-square test.A P value of <0.05 was considered as statistically significant difference . Result There were no significant differences between two groups in terms of operation time and volume of drainage .In regard to complications , there was no statistically significant difference of transientrecurrent laryngeal nerve injuries and hypoparathyroidism . No patient developed hematoma , permanent recurrent laryngeal nerve injuries and hypoparathyroidism in both groups . Conclusion Both Ligasure and Harmonic Focus were safe and efficient for thyroid sugery , with similar clinical outcome , which surgeons could apply according to their own habit .
