彩色多普勒超声定位技术在幕上深部胶质瘤显微手术中的临床应用

摘    要:目的探讨彩色多普勒超声定位技术在颅内胶质瘤手术中的应用价值。方法开颅后暴露硬脑膜,进行彩色多普勒超声超检查,确定病变部位、大小、范围、病变内部及周围血供情况,选择最佳手术路径,在显微镜下切除病变,然后再应用彩色多普勒超声对手术效果进行评估。结果手术前后影像学资料对比显示36例全部定位准确。其中16例显微镜下全切除,12例次全切除,8例部分切除。术后4例出现神经功能缺损加重,3例术前偏瘫短期内加重,1例运动性失语短期内加重,其余患者神经功能保留良好。结论术中颅脑彩色多普勒超声可实时显示病变的部位和范围,有助于病变的术中定位和手术路径的选择,准确的切除病灶,最大程度减少手术的副损伤,具有重要的临床应用价值。

Abstract:Objective To explore the effects of color Doppler ultrasonography in microsurgeries for deep supratentorial glioma. Methods Before removing the tumor under a microscope, color Doppler ultrasonography was used to locate the right position of tumor epidurally or subdurally. Results A comparison between the images of pre- and post-operation showed that tumors were right positioned in 36 cases. 16 cases had total resection under microscopy, 12 had subtotal resection, and 8 had partial resection. After the operations, neurological impairment was aggravated in 4 cases, hemi-paralysis was aggravated and recovered later in 3 cases, and motor aphasia was aggravated in 1 case for a short time. Conclusion Color doppler ultrasonography can be used to locate the position of tumors and decrease the side-effects to a large degree.
