子宫内膜异位症患者血清CA125及CA19-9联合测定的临床评价

目的探讨血清CA125、CA19-9单独及联合测定对子宫内膜异位症(内异症)的诊断价值。方法测定内异症患者103例,子宫腺肌症组27例,子宫肌瘤41例及卵巢浆液性囊腺瘤25例血清CA19-9和CA125。结果内异症、子宫腺肌症、子宫肌瘤、卵巢浆液性囊腺瘤血清CA125阳性率分别为53.4%,57.7%,7.3%和16.0%。内异症、子宫腺肌症、子宫肌瘤CA19-9阳性率分别为48.5%,15.4%,5.7%。本组内异症Ⅰ+Ⅱ及Ⅲ+Ⅳ期患者CA125阳性率分别为34.8%及68.4%,CA19-9阳性率分别为32.6%及61.4%。内异症CA125测定的敏感性、特异性分别为53.4%、96.1%,CA19-9测定的敏感性,特异性分别为48.5%、92.1%,CA125诊断敏感性高于CA19-9,但差异无显著性(P>0.05)。子宫腺肌症CA125及CA19-9的敏感性分别为57.7%和15.4%,有显著性差异(P<0.05)。联合测定的阳性率71.8%,明显高于单独CA125及CA19-9测定阳性率(P<0.05)。结论血清CA125及CA19-9在内异症患者中均明显升高,二者联合测定可进一步提高诊断的敏感性。

Evaluation of serum CA125 and CA19-9 in Patients with endometriosis

Objective To investigate the diagnostic values of CA125 or/and CA19-9 in the serum of Patients with endometriosis.Methods serum CA125 and CA19-9 were measured in 103 endometriosis,27 had adenomyosis,41 leiomyoma of uterus and 25 ovarian serous cystadenoma respectively.Results The CA125 positive rates for patients with endometriosis,adenomyosis,leiomyoma of uterus and ovarian serous cystadenoma were 53.4%,57.7%,7.3% and 16.0% respectively.The CA19-9 positive rates for patients with endometriosis,adenomyosis and leiomyoma of uterus were 48.5%,15.4% and 5.7% respectively.In patients with endometriosis stage Ⅰ+Ⅱ and stage Ⅲ+Ⅳ,the CA125 positive rates were 34.8% and 68.4% respectively,the CA19-9 positive rates were 32.6% and 61.4% respectively.The sensitivity,specificity of CA125 for endometriosis were 53.4%?96.1% respectively,the sensitivity,specificity of CA19-9 for endometriosis wered 48.5%?92.1% respectively,the diagnostic sensitivity of CA125 was slightly higher than that of CA19-9,but the differences were not significant(P>0.05).The sensitivity of CA125 and CA19-9 for adenomyosis were 57.70% and 15.4%,the difference was significant(P<0.05).The concomitant positive rates of CA125 and CA19-9 was 71.8%,which was significantly higher than the of positive rate of CA125 and CA19-9 alone.(P<0.05).Conclusion Serum CA125 and CA199 are elevated in patients with endometriosis,combined monitor of the CA125 and CA19-9 can raise the sensitivity of diagnosis for endometriosis.
