摘    要:本研究中的一例无自主呼吸的深昏迷患者突然恢复"自主呼吸",但意识及神经系统体征却无好转,食道压监测提示为呼吸机自动触发。通过排查,发现由于呼气过滤器与集水器连接处螺纹口松动所致,重新连接紧密后,自动触发消失,患者无自主呼吸。可见食道压监测可以更加准确地识别人机失调,有助于更加准确地评估临床病情。

Abstract:A spontaneous breath was found in a patient who was diagnosed as cerebral infarction and had no apparent neurological function. Esophageal pressure monitoring showed that the breaths were not triggered by the patient effort, so it was identified as ventilator auto-triggering. It turned out to be caused by a leakage of the breathing circuit, and the auto-triggering disappeared after the leakage was solved. Esophageal pressure monitoring could help to identify patient-ventilator asynchrony and improve clinical judgment and decision-making.
