两种不同入路方式行腹腔镜下右半结肠癌根治术临床效果对比

目的:对比腹腔镜下两种不同入路方式行右半结肠癌根治术的临床效果。方法:前瞻性选取2016年1月至2018 年10月我院120例行腹腔镜右半结肠癌根治术的患者进行研究，按所选入路方式不同分为观察组和对照组各60例，观察组采用尾侧入路、对照组采用中间入路。利用统计软件SPSS 19.0进行数据处理，观察分析两组患者围手术期指标及术后并发症，评价腹腔镜下两种入路的临床效果。结果:两组患者性别、年龄、术前TNM分期及肿瘤部位等一般资料无明显差异（P＞0.05）；与对照组相比，观察组手术时间短、术中出血量少、血管损伤率低，组间差异有统计学意义(P＜0.05)；术中淋巴结清扫数目、中转开腹率，术后恢复排气/排便时间、拔除引流管时间、住院时间及术后并发症发生率较对照组比较均无统计学意义(P＞0.05)。结论:与中间入路相比，尾侧入路行腹腔镜下右半结肠癌根治术临床效果更好。

Comparison of the clinical effects of laparoscopic radical resection of right colon cancer by two different approaches

Objective:To compare the clinical effects of laparoscopic radical resection of right colon cancer by two different approaches.Methods:A prospective study was conducted among 120 patients who underwent laparoscopic right hemicolon cancer radical resection in our hospital from January 2016 to October 2018.The patients were divided into the observation group and the control group with 60 patients in each group according to the method of the selected approach.The observation group adopted the caudal approach and the control group adopted the intermediate approach.Statistical software SPSS 19.0 was used for data processing to observe and analyze the perioperative indicators and postoperative complications of the two groups of patients,and to evaluate the clinical efficacy of the two approaches under laparoscope.Results:There were no significant differences in gender,age,preoperative TNM stage and tumor site between the two groups (P＞0.05).Compared with the control group,the observation group had shorter operation time,less intraoperative blood loss and lower vascular injury rate,and the difference between the two groups was statistically significant (P＜0.05).The number of intraoperative lymph node dissection,the conversion rate of laparotomy,the postoperative recovery time of exhaust/defecation,the removal of drainage tube,the length of hospital stay and the incidence of postoperative complications were not statistically significant compared with the control group (P＞0.05).Conclusion:Compared with the middle approach,the caudal approach is more effective for right colon cancer radical resection under laparoscope.
