18F-FDG PET/CT最大标准化摄取值与肺癌临床相关因素的关系研究

摘    要:目的:探讨18F-FDG PET/CT最大标准化摄取值(standard uptake value maximum,SUVmax)与肺癌临床相关因素的关系研究,为肺癌的综合评估提供依据。方法:回顾分析2015年1月至2018年8月在内蒙古自治区人民医院行18F-FDG PET/CT检查,且经病理证实为肺癌患者的临床资料。根据临床病理特征分组,分析SUVmax与性别、年龄、肿瘤位置、肿瘤最大径、病理类型及临床分期之间的关系。结果:符合入选标准的共有99例患者,其中腺癌61例,鳞癌29例,小细胞肺癌9例。独立样本t检验结果显示不同年龄(P=0.182)及肿瘤位置(P=0.536)SUVmax平均值差异无统计学意义;不同性别及肿瘤最大径SUVmax平均值差异有统计学意义,且男性患者的SUVmax高于女性(P=0.000),肿瘤最大径>3 cm者SUVmax高于≤3 cm者(P=0.000)。单因素方差分析结果显示不同病理类型(P=0.000)及临床分期(P=0.000)SUVmax平均值差异有统计学意义。相关性分析显示SUVmax与性别(P=0.000)、肿瘤最大径(P=0.000)、临床分期(P=0.000)及病理类型(P=0.000)存在相关性;SUVmax与年龄(P=0.182)及肿瘤位置(P=0.431)无相关性。多重线性回归分析结果显示肿瘤最大径(P=0.000)、病理类型(P=0.002)及临床分期(P=0.000)是影响SUVmax的独立因素,而性别、年龄及肿瘤位置对SUVmax无独立影响作用。结论:性别、肿瘤最大径、病理类型及临床分期可影响肺癌的SUVmax,肿瘤最大径、病理类型及临床分期是影响SUVmax的独立因素,SUVmax可能有助于判断肺癌的病理类型及预测其远处转移,对临床治疗方案的选择和预后判断可给予一定的指导。

Abstract:Objective:To explore the relationship between 18F-FDG PET/CT standard uptake value and clinical factors associated with lung cancer,and to provide the basis for the comprehensive evaluation of lung cancer.Methods:We retrospectively reviewed the clinical date of patients underwent 18F-FDG PET/CT examination in People's Hospital of Inner Mongolia Autonomous Region from January 2015 to August 2018,and confirmed as lung cancer by pathology in the later stage.Subjects were divided into different groups according to clinicpathologic results,and correlations between different gender,age,location,maximum diameter of the tumor,pathological type and clinical stages were analyzed.Results:A total of 99 patients were included in this study,including 61 with adenocarcinoma,29 with squamous cell carcinoma and 9 with small cell lung carcinoma.Independent sample T test demonstrated that there was no statistical significance in SUVmax among different ages(P=0.182) and tumor location(P=0.536).There was statistical significance in SUVmax among different gender(P=0.000)and tumor size(P=0.000).In addition,SUVmax was larger in male patients than female,and SUVmax was larger in tumor size larger than 3 cm.One way Anova showed that there was statistical significance in SUVmax among different pathological types(P=0.000)and clinical stages(P=0.000).Correlation analysis indicated that SUVmax was correlated with gender(P=0.000),tumor size(P=0.000),clinical stages(P=0.000) and pathological types(P=0.000),while SUVmax was no correlated with age(P=0.182)and location(P=0.431).Multiple linear regression analysis showed that tumor size,pathological types and clinical stages were the independent influence on SUVmax,while gender,age and location had no independent influence on SUVmax.Conclusion:The SUVmax of lung cancer was affected by gender,tumor size,pathological types and clinical stages.Tumor size,pathological types and clinical stages were the independent influence on SUVmax.SUVmax may contribute to evaluate the histological types of non-small cell lung cancer and predict metastasis,thus can provide reference for options of therapeutic scheme and prognosis.
