多层螺旋CT与内镜超声检查对胃癌的术前评估价值比较

目的:比较多层螺旋CT与内镜超声检查对胃癌的术前评估价值。方法:采用回顾性总结研究方法，选择2014年6月至2018年1月在我院普外科手术切除并经病理证实的胃癌患者66例作为研究对象，记录所有患者的多层螺旋CT与内镜超声检查结果，并与病理检查结果进行对比分析。结果:在66例患者中，T分期为T1期6例，T2期14期，T3期30例，T4期16例；N分期为N0期16例，N1期14期，N2期36例；M分期为M0期46例，M1期20期。内镜超声对TNM分期判断准确率分别为92.4%、92.4%和93.9%，CT对TNM分期判断准确率分别为93.9%、97.0%和98.5%，CT都好于内镜超声，但是对比差异无统计学意义(P＞0.05)。结论:多层螺旋CT与内镜超声检查在胃癌TNM分期的术前评估都有较高的准确率，在临床上可结合两者方法共同使用，以提高诊断效果。

Comparison of preoperative evaluation of gastric cancer by multi-slice spiral CT and endoscopic ultrasonography

Objective:To compare the preoperative evaluation of gastric cancer by multi-slice spiral CT and endoscopic ultrasonography.Methods:With a retrospective summary of research methods,from June 2014 to January 2018,6 cases of gastric cancer patients confirmed by pathology in the Department of general surgery in our hospital were selected as the research object.All patients with multi-slice spiral CT and endoscopic ultrasound examination results were recorded,and the results were compared with pathological examination.Results:In the 66 patients,there were 6 cases of T1 stage,14 cases of T2 stage,30 cases of T3 stage,16 cases of T4 stage.There were 16 cases of N0 stage,14 cases of N1 stage,36 cases of N2 stage.There were 46 cases of M0 phase,20 cases of M1 stage.The accuracy rate of endoscopic ultrasonography for TNM staging were 92.4%,92.4% and 93.9%,respectively.The accuracy rate of CT for TNM staging were 93.9%,97.0% and 98.5%.CT was better than endoscopic ultrasonography,but the difference were not statistically significant (P＞0.05).Conclusion:The multi-slice spiral CT and endoscopic ultrasonography have high accuracy in preoperative evaluation of TNM staging of gastric cancer.They can be used in combination with both methods in clinical practice,so as to improve the diagnostic effect.
