血清TGF-β、VTN水平与成人脑胶质瘤病理分级及预后的关系

摘    要:目的 探讨外周血转化生长因子-β(TGF-β)、玻连蛋白(VTN)水平与成人脑胶质瘤病理分级及预后的关系。方法 选取2015年1月至2016年1月手术治疗的胶质瘤98例(高级别53例,低级别45例)和同期健康体检者50例(对照组)为研究对象,术前1 d、术后 7 d检测血清TGF-β、VTN水平。术后随访3年。结果 术前1 d胶质瘤病人血清TGF-β、VTN水平均显著高于对照组(P<0.05),而且术前1 d血清TGF-β(r=0.734,P<0.05)、VTN(r=0.717,P<0.05)水平与胶质瘤病理分级呈正相关。术后3年,98例胶质瘤中,死亡69例,存活29例;存活病人术前血清TGF-β、VTN水平均显著低于死亡病人(P<0.05);ROC曲线分析结果显示术前1 d血清TGF-β、VTN水平预测术后3年病人死亡风险的最佳截断值分别为350.39 pg/ml、86.12 μg/ml。结论 胶质瘤病人血清TGF-β、VTN水平与肿瘤病理分级正相关,而且血清TGF-β、VTN水平升高提示病人预后不良。

Abstract:Objective To analyze relationship of serum levels of transforming growth factor-β (TGF-β) and vitronectin (VTN) with the pathological grade of the tumors and prognosis in adult patients with cerebral gliomas. Methods The serum levels of TGF-β and VTN were determined in the 98 adult patients with cerebral gliomas who were treated by surgery in our hospital from January, 2015 to January, 2016 and 50 healthful subjects. Of 98 patients with cerebral gliomas, 11 had WHO grade I gliomas, 42 grade II, 27 grade III and 18 grade IV. The relationships of postoperative serum level of TGF-β and VTN with WHO pathological grade of brain gliomas and the patients prognosis were analyzed. Results The serum levels of TGF-β and VTN were significantly higher in the patients with WHO grades Ⅲ and Ⅳ brain gliomas than those in the patients with WHO grades I and Ⅱ (P<0.05). The serum levels of TGF-β (r=0.734, P<0.05) and VTN (r=0.717, P<0.05) were positively correlated with WHO pathological grade of brain glioma. The cut-off values of TGF-β and VTN for predicting prognosis in the patients with gliomas were 350.39 pg/ml and 86.12 μg/ml, respectively. Conclusions The serum levels of TGF-β and VTN in the patients with brain gliomas are positively correlated with WHO pathological grade in the tumors. The increase of the serum TGF-β or VTN levels suggests poor prognosis in the patients with cerebral gliomas.
