丁苯酞注射液对脑梗死患者血清细胞凋亡因子水平的影响

摘    要:目的探究丁苯酞注射液对脑梗死患者血清细胞凋亡因子水平的影响。方法选取蚌埠医学院第一附属医院神经内科2017年1-12月收治的急性脑梗死患者106例,按照随机数字表法分为对照组及观察组,每组53例。患者入院后均给予常规治疗以及双联抗血小板治疗,在此基础上观察组患者联合丁苯酞注射液治疗。比较2组患者治疗总有效率,治疗前后2组患者细胞凋亡因子Bcl-2、CytC水平,血清炎症因子TNF-α、IL-6水平以及美国国立卫生研究院卒中量表(NIHSS)、Barthel指数和改良Rankin量表(MRS)评分。结果观察组治疗总有效率(90.57%)明显高于对照组(73.58%),2组差异有统计学意义(χ^2=5.194,P=0.023)。治疗2周后,观察组患者Bcl-2水平明显升高,CytC水平明显降低,与对照组比较差异均有统计学意义(均P<0.05);2组患者血清TNF-α、IL-6水平均明显降低,且观察组患者血清TNF-α、IL-6水平明显低于对照组(均P<0.05)。观察组患者治疗后4周NIHSS、MRS评分明显低于对照组,Barthel指数评分明显高于对照组(均P<0.05)。结论丁苯酞注射液治疗急性脑梗死患者临床疗效显著,可有效降低血清炎症因子水平,改善脑细胞凋亡因子水平,改善患者神经功能评分。

Abstract:Objective To investigate the effect of butylphthalide injection on the serum level of apoptosis-related factors in patients with cerebral infarction. Methods Total 106 patients with acute cerebral infarction admitted to our hospital from January 2017 to December 2017 were randomly divided into control group and observation group, with 53 cases in each group. All patients received conventional therapy and dual antiplatelet therapy after admission. On this basis, the patients in the observation group were additional administrated with butylphthalide injection. The total effective rate was compared between the two groups. The levels of Bcl-2 and CytC, serum inflammatory factors TNF-α, IL-6 and the national institutes of health stroke scale (NIHSS), Barthel index and the modified Rankin scale (MRS) scores were measured before and after the treatment. Results The total effective rate of the observation group (90.57%) was significantly higher than that of the control group (73.58%), with a statistically significant (χ2=5.194, P=0.023). After 2 weeks of treatment, the Bcl-2 level elevated and the level of CytC decreased significantly in the observation group, and the difference was statistically significant when compared with the control group (P<0.05). The serum levels of TNF-α and IL-6 were significantly decreased in both groups, especially in the observation group (P<0.05). After 4 weeks of treatment, the NIHSS and MRS scores of the observation group were significantly lower than those of the control group, but The Barthel index score was significantly higher in the observation group than that of the control group, and the difference was statistically significant (P<0.05). Conclusion Butylphthalide injection has a significant clinical effect on patients with acute cerebral infarction, which can effectively reduce serum inflammatory factor levels and improve brain cell apoptosis-related factors levels, improve neurological function scores, and is worthy of clinical use.
