摘    要:目的探讨Bcl-2、P16蛋白在食管鳞癌及癌前病变中的表达及其与临床病理参数的关系。方法应用免疫组织化学方法(Envision法)检测Bcl-2、P16蛋白在74例食管鳞癌、上皮内瘤变及癌旁正常黏膜组织中的表达情况。结果Bcl-2蛋白在食管正常黏膜组织、高级别上皮内瘤变及鳞癌组织中的阳性表达率分别为1.4%、43.5%、66.2%,各组之间差异有统计学意义(P<0.01),Bcl-2表达与组织学分级、淋巴结转移有相关性。P16蛋白在食管正常黏膜组织、高级别上皮内瘤变及鳞癌组织中的表达分别为40.4%、20.3%、20.3%,P16蛋白的表达在正常黏膜组与其余各组之间差异有统计学意义(P<0.01),但在上皮内瘤变组和食管鳞癌组之间P16蛋白表达差异无统计学意义。结论Bcl-2与P16在食管鳞癌发生发展的不同时期起作用,P16缺失出现在食管癌发生的早期阶段。Bcl-2与食管鳞癌的发生及进展有关,检测P16和Bcl-2的表达对食管鳞癌的早期诊断、判断预后有参考价值。

Abstract:Objective To investigate the expression and significance of Bcl-2 and P16 protein in esophageal squamous cell carcinoma (ESCC) and precancerous lesions and the correlation with clinical pathologic parameters. Methods Immunohistochemistry assay (Envision method) was employed to examine the expression of Bel-2 and P16 protein in ESCC tissues, intraepithelial neoplasia and normal esophageal mucosa. Results The Bcl-2 protein expres- sion differed statistically (P〈0.01) among normal esophageal mucosa (1.4%), high-grade intraepithelial neoplasia (43.5%) and ESCC tissues (66.2%). The expression of Bel-2 correlated with the differentiation of carcinoma and lymph node metastasis (P〈0.05). Compared with normal esophageal mucosa (40.4%), the level of P16 protein expression was appreeiably lower (both P〈0.01) in high-grade intraepithelial neoplasia (20.3%) and ESCC tissues (20.3%). There was no remarkable difference in P16 protein expression between ESCC and precancerous lesion group (P〉0.05). Conclu- sion Bel-2 and P16 play distinct roles at different developmental stages of ESCC. The loss of P16 takes place in ear- ly stage ESCC while Bcl-2 is associated with the development and progression of ESCC. Assay of Bcl-2 and P16 ex- pression may serve as a reference for early diagnosis and prognosis of ESCC.
