尿Ⅳ型胶原检测在系膜增殖性肾小球疾病中的应用

目的 评价双抗体夹心ELISA法检测尿Ⅳ型胶原(Ⅳ-c)对肾脏疾病患者系膜细胞增殖、细胞外基质分泌和肾小球硬化的意义。方法 比较分析该方法测定15名正常人和51名肾病患者的尿Ⅳ-c。结果 双抗体夹心ELISA法的灵敏度为1.2ng/ml;检测范围3.2～200ng/ml。原发性系膜增殖性肾小球肾炎(MsPGN)组和非MsPGN组尿Ⅳ-c、尿白蛋白(Alb)显著高于正常对照组(P<0.01),MsPGN组尿Ⅳ-c显著高于非MsPGN组(P<0.01)。MsPGN、糖尿病肾病和IgA肾病患者的尿Ⅳ-c明显高于正常对照和其它肾病患者(P<0.05,P<0.01)。结论 双抗体夹心FLISA法检测尿Ⅳ-c简便、灵敏。

Enzyme Immunoassay for Urinary Ⅳ Collagen and Its Application in Membranoproliferative Glomerulopathy

Objective To develope enzyme immunoassay for urinary IV collagen in order to evaluate proliferatoin of mesangial cell and accumulation of extracellular matrix and renal fibrosis in nephropa-thy patients. Methods Assay the level of urinary IV collagen (IV - c) , urinary albumin in 15 normal persons was assayed and 51 nephropathy patients. Results The detection limit of this assay was 1.2ng/ml, The working range was 3.2-200ng/ml. The levels of urinary IV - c and urinary albmumin were significantly higher in the membranoproliferative - glomerulonephropathy (MsPGN) group and the non - MsPGN group than normal controls (P <0.01) , The urinary IV - c was significantly higher in the MsPGN group than the non - MsPGN group (P < 0. 01 ). The levels of urinary IV - c were much higher in diabetic nephropathy ( DN) and IgA nephropathy ( IgAN ) patients than the normal group and the other nephropathy group (P <0. 05 , P < 0. 01). Conclusion We have developed a sensitive and simple ELISA for urinary IV -c.
