bcl—2,c—erbB—2在胃粘膜癌变过程中的表达及意义

目的：通过检测bcl-2,c-erbB-2基因在胃粘膜良恶生病变中的表达，探讨其对提高胃癌诊断率的作用。方法：采用免疫组化SABC法，检测bcl-2,c-erbB-2基因在98例胃癌，40例不典型增生和20例正常胃粘膜中的表达，结果a,bcl-2在正常胃粘膜中有弱阳性表达（10．0％），在不典型增生中为50％，在胃癌中为44．9％，不典型增生与胃癌中的表达均显著高于正常胃粘膜（P＜0．05），bcl-2表达与胃癌的分化程度有关（P＜0．05），低分化者bcl-2表达率高，与淋巴结转移，浸润深度无关（P＞0．05），b.c-erbB-2在胃癌中的阳性表达率为56．1％，其中中早期胃癌中的表达低（16．7％），在重度不典型增生和进展期胃癌中的表达高，分别为80％，58．7％，二者与早期胃癌比较有统计学意义（P＜05），c-erbB-2表达与浸润深度，淋巴结转移有关（P＜0．05），与分化程度无关（P＞0．05），结论： -2,－B-2c参与胃粘膜的癌变过程，联合检测bcl-2,c-erbB-2可能作为胃癌高危人群筛选的有效指标。

THE EXPRESSION OF bcl-2,c-erbB-2 AND ITS SIGNIFICANCE IN THE DEVELOPMENT OF GASTRIC CANCER

Objective:To detect the expression of bcl-2 and c-erbB-2 in gastric mucosa of benign and malignant lesions and estimate its diagnosis value. Methods:Using S-P method. Results:a. The positive rate of bcl-2 expression was 10%, 50% and 44.9% in normal gastric mucosa, atypical hyperplasia and gastric cancer respectively. Significant difference was seen between the two groups and normal gastric mucosa(P<0 05). There was relationship between bcl-2 expression and differentiation of gastric cancer, but bcl-2 expression was not related with lymphnode metastasis(LNM) and depth of infiltration. b. The expression of c-erbB-2 gene was found in gastric cancer (56.1%) and low in early stage(16.7%), but high in the advanced stage(58.7%), the heavy atypical hyperplasia(80%), there was a significant difference between the two groups and the early stage of gastric cancer (P<0 05). The expression of c-erbB-2 had something to do with LNM depth of infiltration(P<0 05) and nothing to do with differentiation(P>0 05). Conclusion:bcl-2 and c-erbB-2 may be involved in the course of stomach carcinogenesis, detecting the expression of bcl-2 and c-erbB-2 may be a useful marker for identifying patients at high risk of developing gastric cancer.
