阿托伐他汀对2型糖尿病患者血脂调节及抗动脉粥样硬化的作用

目的:探讨阿托伐他汀对初诊2型糖尿病病患者血脂的调节及对早期动脉粥样硬化的防治作用。方法:采用随机、开放、前瞻性研究方法,选择无大血管病变、血脂轻度异常的初诊2型糖尿病患者155例,随机分为阿托伐他汀治疗组(治疗组)80例和非阿托伐他汀治疗组(对照组)75例,在控制血糖、血压的基础上,给予阿托伐他汀治疗,所有患者的治疗期均为12个月。结果:随访12个月后,治疗组空腹血糖(FBS)、餐后2h血糖(2h FBS)、糖化血红蛋白(HbA1C)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL-C)、高敏C反应蛋白(hs-CRP)、颈总动脉IMT增厚水平明显低于对照组;治疗后,两组之间高密度脂蛋白(HDL-C)无统计学差异;直线相关分析显示治疗组TC、TG、LDL-C和hs-CRP的下降与阿托伐他汀的治疗成正相关,颈总动脉IMT增厚与阿托伐他汀的治疗成负相关。结论:阿托伐他汀对初诊2型糖尿病患者血糖、血脂均有调节作用,并可能具有改善早期动脉粥样硬化、调节炎症反应的作用。

Modulation Effect on Dislipidemia and Atherosclerosis by Atorvastatin in Newly Diagnosed Type 2 Diabetes Patients

Objective To explore the modulation effect of Atorvastatin on dislipidemia and atherosclerosis in newly diagnosed type 2 diabetes patients.Methods 155 newly diagnosed type 2 diabetes patients were randomly to receive Atorvastatin therapy(40mg/d,n=80) and conventional treatment group(n=75);the mean follow-up period was 12 months.Results Under the same control of blood glucose and blood pressure,the serum levels of FBS,2h FBS,HbA1C,TC,TG,LDL-C,hs-CRP and Carotid intima-medial thickness(IMT) were significantly decreased in Atorvastatin therapy group.The correlation analysis revealed the decreased levels of TC,TG,LDL-C,hs-CRP and IMT were related to the Atorvastatin therapy.Conclusion Atorvastatin therapy is effective on modulating dislipidemia in newly diagnosed type 2 diabetes patients and is related to improve the arteriosclerosis and inflammatory reaction.
