右美托咪定预防气管插管全身麻醉患者拔管期心血管反应50例

摘    要:目的观察右美托咪定预防气管插管心血管应激反应的疗效。方法选择美国麻醉医师协会分级(ASA)Ⅰ~Ⅱ级择期行腹腔镜胆囊切除术患者100例,随机分为右关托咪定组(D组)和对照组(N组),各50例。两组患者均采用相同的全身麻醉诱导和维持,麻醉诱导前10min,D组静脉注射右美托咪定1P,g/kg,N组注射相同容量的0.9%氯化钠注射液,注射时间10min。观察并记录两组诱导前、拔管时及拔管后1,5,10min时的收缩压(SBP)、舒张压(DBP)、心率(HR)。结果D组拔管期心血管反应稳定,D组收缩压、舒张压、心率在拔管前1min、拔管时及拔管后1min均明显低于N组(P〈0.05)。与同组诱导前比较,N组拔管前lrain、拔管时、拔管后1rain的SBP。DBP,HR均升高(P〈0.05)。结论右美托咪定能有效减轻气管插管全身麻醉患者拔管期心血管应激反应。

Abstract:Objective To observe the effects of dexmedetomidine in prventing cardiovascular response of general anesthesia patient dur- ing decannulation period. Methods One hunder patients with ASA class Ⅰ- Ⅱ undergoing laparoscopic cholecystectomy were randomly divided into dexmedetomidine group(group D) and control group (group N), 50 cases in each group. The patients in the two groups were treated by the same anesthesia induction and maintenance, 10 minutes before induction of anesthesia, the patients in group D were given intravenous injection of dexmedetomidine 1 p,g/kg, while the group N were given intravenous injection of same amount of normal saline. The injection time of patients in two groups were 10 min. 1 rain before extubation, at the time of extubation, and 1, 5, 10 inin af- ter extubation, systolic blood pressure(SBP), diastolic blood pressure(DBP), heart rate(HR)were observed and recorded. Results Cardio- vascular response in group D is stable in the extubation period. SBP, DBP, HR in group D at l minute before extubation, at the time of extubation and 1 rain after extubation were significantly lower than those in the group N( P 〈 0.05). Compared with the same group be- fore induction, SBP, DBP, HR in group N at 1 minute before extubation, at the time of extubation and 1 min after extubation were sig- nificantly higher( P 〈 0.05). Conclusion Dexmedetomidine can effectively decrease the cardiovascular responses of general anesthesia patient in extubation period.
