腹腔镜内侧径路法乙状结肠癌切除术

目的 探讨腹腔镜内侧径路法治疗乙状结肠癌的手术方法。方法 分析2001年4月～2002年11月施行的13例腹腔镜乙状结肠癌切除术患者的手术方法，采用内侧径路法，即先处理肠系膜根部及中枢侧淋巴、血管，后处理病变肠管。结果 13例患者均成功完成腹腔镜手术，平均手术时间130min，术中平均出血30mL，平均术后住院日7d，无手术并发症和死亡发生，随诊6～21个月，未发现局部或全身肿瘤复发、转移。结论 只要严格掌握手术指征，合理选择手术方式，腹腔镜内侧径路法治疗结肠肿瘤是安全、有效的。

Application of median approach in laparoscopic colectomy of sigmoid colon cancer

Objective] To evaluate the median approach in the laparoscopic colectomy of sigmoid colon cancer. Methods] Thirteen patients with sigmoid colon cancer underwent laparoscopic colectomy from April 2001 to November 2002. Median incision of retroperitonium was made first for dissection of mesenteric vesolymphatic pedicle. Then the bowel with tumor was isolated and excised, Results] All 13 patients recovered with no complications. The mean operative time was 130 min, the mean blood loss was 30ml, and the mean postoperative hospital stay was 7 days. All patients were followed up from 6 to 21 months, No local recurrence or distant metastasis was found. Conclusions] Laparoscopic colectomy of sigmoid colon cancer with median approach is safe and effective when indicated.
