Cortactin、MMP-2和MMP-9蛋白在肝细胞肝癌组织中的表达及临床意义

目的探讨皮动蛋白(Cortactin)、基质金属蛋白酶-2(MMP-2)和基质金属蛋白酶-9(MMP-9)在肝细胞肝癌(HCC)组织中的表达及临床意义。方法采取回顾性研究,选取2017年1月至2019年12月空军特色医学中心病理科保存的HCC组织110例(HCC组),同时收集正常肝组织50例作为对照组。采用免疫组织化学染色法检测Cortactin、MMP-2和MMP-9蛋白表达。比较HCC组和对照组组织中Cortactin、MMP-2和MMP-9蛋白表达;分析HCC患者Cortactin、MMP-2和MMP-9蛋白表达与临床病理关系;分析HCC患者组织中Cortactin表达与MMP-2、MMP-9表达相关性;分析HCC患者组织中Cortactin、MMP-2和MMP-9蛋白表达与无进展生存时间的关系。结果HCC组Cortactin、MMP-2和MMP-9蛋白阳性表达率分别为61.82%、51.82%和80.91%,明显高于对照组(30.00%、22.00%、22.00%),差异有统计学意义(P<0.05)。TNM分期Ⅲ~Ⅳ期、有淋巴结转移和门静脉侵袭的HCC组织Cortactin阳性表达率分别为85.00%、81.63%和83.33%,明显高于Ⅰ~Ⅱ期、无淋巴结转移、无门静脉侵袭(48.57%、45.90%、51.35%),差异有统计学意义(P<0.05);TNM分期Ⅲ~Ⅳ期、有淋巴结转移的HCC组织MMP-2阳性表达率分别为80.00%和75.51%,明显高于Ⅰ~Ⅱ期和无淋巴结转移(35.71%、32.79%),差异有统计学意义(P<0.05);TNM分期Ⅲ~Ⅳ期、有淋巴结转移的HCC组织MMP-9阳性表达率分别为95.00%和95.92%,明显高于Ⅰ~Ⅱ期和无淋巴结转移(72.86%、68.85%),差异有统计学意义(P<0.05)。HCC组织Cortactin表达与MMP-2、MMP-9表达呈正相关(r s=0.590、0.332,P<0.05);MMP-2和MMP-9表达呈正相关(r s=0.411,P<0.05)。Cortactin阳性表达者无进展生存时间为14个月,明显短于Cortactin阴性表达者20个月(P<0.05);MMP-2阳性表达者无进展生存时间为13个月,明显短于MMP-2阴性表达者21个月(P<0.05);MMP-9阳性和阴性表达者无进展生存时间比较,差异无统计学意义(P<0.05)。结论HCC组织中Cortactin、MMP-2和MMP-9蛋白表达上调,与临床分期、淋巴结转移及门静脉侵袭有关,同时3者间表达存在正相关关系。

Expression of Cortactician,MMP-2 and MMP-9 proteins in hepatocellular carcinoma tissues and their clinical significance

Objective To investigate the expression and clinical significance of Cortactician(Cortactician),matrix metalloproteinase-2(MMP-2)and matrix metalloproteinase-9(MMP-9)in hepatocellular carcinoma(HCC).Methods From January 2017 to December 2019,110 cases of HCC tissues(HCC group)preserved in the Pathology Department of the Air Force Special Medical Center were selected retrospectively,and 50 cases of normal liver tissues were collected as control group.The protein expressions of Cortactin,MMP-2 and MMP-9 were detected by immunohistochemistry.Cortactin,MMP-2,and MMP-9 protein expressions in HCC group and control group were compared;Cortactin,MMP-2,and MMP-9 protein expression in HCC patients were analyzed and their relationship with clinicopathology was analyzed;Cortactin expression in tissues of HCC patients was correlated with MMP-2.The correlation of MMP-9 expression was analyzed;the relationship between Cortactin,MMP-2 and MMP-9 protein expression in HCC patient tissues and progression-free survival time was analyzed.Results The positive expression rates of cortactin,MMP-2 and MMP-9 in HCC group were 61.82%,51.82%and 80.91%,which were significantly higher than those in control group(30.00%,22.00%,22.00%),the differences were statistically significant(P<0.05).The positive expression rates of Cortatin in TNM stageⅢ-Ⅳ,lymph node metastasis and portal vein invasion were 85.00%,81.63%and 83.33%,which were significantly higher than those in stageⅠ-Ⅱ,lymph node metastasis and portal vein invasion(48.57%,45.90%,51.35%),the differences were statistically significant(P<0.05);The positive expression rates of MMP-2 in TNM stageⅢ-Ⅳand lymph node metastasis were 80.00%and 75.51%,which were significantly higher than those in stageⅠ-Ⅱand without lymph node metastasis(35.71%,32.79%)(P<0.05);The positive expression rates of MMP-9 in TNM stageⅢ-Ⅳand lymph node metastasis were 95.00%and 95.92%,which were significantly higher than those in stageⅠ-Ⅱand without lymph node metastasis(72.86%,68.85%)(P<0.05);The expression of MMP-2 and MMP-9 was positively correlated(r s=0.411,P<0.05).The progression free survival time was 14 months,which was significantly shorter than that in patients with negative expression of cortactin 20 months(P<0.05);The progression free survival time of MMP-2 positive group was 13 months,which was significantly shorter than that of MMP-2 negative expression group 21 months(P<0.05);There was no significant difference in progression free survival time between MMP-9 positive and negative expression groups(P<0.05).Conclusion The up-regulated expression of Cortatin,MMP-2 and MMP-9 in HCC tissues is correlated with clinical staging,lymph node metastasis and portal vein invasion,and the expression of these proteins is positively correlated.
