Effect of laparoscopic D2 radical gastrectomy on the levels of pepsinogen,gastrin-17 and serum CA724 in patients with locally advanced distal gastric cancer

Objective To observe the effect of laparoscopic D2 radical operation on serum pepsinogen(PG),gastrin-17(G-17)and carbohydrate antigen 724(CA724)levels in patients with locally advanced distal gastric cancer.Methods A total of 100 patients with locally advanced distal gastric cancer admitted to Ya'an people's Hospital from May 2017 to may 2020 were prospectively selected and randomly divided into observation group and control group,50 cases in each group.The observation group was given laparoscopic D2 radical resection,and the control group was given traditional D2 radical resection.The operation indexes,serum PG,G-17 and CA724 levels before and after treatment were compared between the two groups,and the incidence of postoperative complications was followed up.Results The operation time[(178.28±14.18 min)],the number of lymph node dissections[(18.38±3.52)pieces]in the observation group were compared with the control group[(180.29±14.93)min,(17.83±3.53)pieces],there was no statistical difference significance(P>0.05);intraoperative blood loss[(164.39±21.14)mL],surgical incision length[(6.02±1.24)cm],postoperative exhaust time[(2.01±0.54)d],hospital stay[(13.25±2.16)d]were significantly less than the control group[(324.29±34.49)mL,(15.39±2.93)cm,(3.19±0.33)d,(18.38±2.04)d],the differences were statistically significant(P<0.05);The level of serum PG I[(98.39±7.32)μg/L]in the observation group at 1 week after operation was significantly higher than that of the control group[(74.63±7.86)μg/L],the levels of G-17[(8.28±0.52)μg/L]and CA724[(8.21±1.35)ng/mL]were significantly lower than those of the control group[(14.04±1.48)μg/L,(17.29±2.45)ng/mL],the differences were statistically significant(P<0.05).The incidence of complications in the observation group(6.00%)was significantly lower than that in the control group(20.00%),the difference was statistically significant(P<0.05).Conclusion Compared with traditional D2 radical gastrectomy,laparoscopic D2 radical gastrectomy in the treatment of locally advanced distal gastric cancer has less trauma,faster postoperative recovery,outstanding effect in improving the level of PGⅠ,G-17 and CA724 levels,less complications,and can be promoted in clinical practice.
