小脑蚓部转移瘤合并局部脑膜癌病的影像学特征

目的 探讨小脑蚓部转移瘤合并邻近脑膜癌病的影像学表现.资料与方法 回顾性分析15例经临床、脑脊液检查证实的小脑蚓部转移瘤合并邻近脑膜癌病患者的资料,着重分析其发生机制与影像学特征.结果 15例表现为小脑蚓部结节状(11例)或斑片状明显强化病灶,病变可扩展至小脑半球,其中囊实性8例均为环状强化,灶周水肿明显.15例小脑蚓部脑膜癌病均为软脑膜一蛛网膜下腔型,仅3例合并后颅凹硬脑膜异常强化.11例合并颅内其他部位转移性病变.结论 小脑蚓部转移瘤合并邻近脑膜癌病有一定的影像学特征,二者合并出现可能与其独特血供有关.

Imaging Features of Vermis Metastases and Local Meningeal Carcinomatosis

Objective To discuss the imaging features of vermis metastases and local meningeal carcinomatosis.Materials and Methods Imaging features of 15 cases with vermis metastases with local meningeal carcinomatosis confirmed by clinically and cerebrospinal fluid examination were retrospectively analyzed.The characteristic imaging features and the mechanism were analyzed.Results The vermis metastases showed nodular(11cases) or patchy obviously enhanced lesions and involved cerebellum.8 Cases had cystic with ring-like enhancement and surrounding edema.The local pial-arachnoid enhancement was found in all 15 patients,dural enhancement was associated only in 3 cases.Additional intracranial metastases were found in 11 patients.Conclusion There are some specific imaging features of vermis metastases and local meningeal carcinomatosis.Concurrence of the parenchymal metastases and local meningeal carcinomatosis may be due to its specific blood supply.
