细胞学结合人乳头瘤病毒DNA检测在宫颈病变诊断中的意义

摘    要:目的探讨液基细胞学(LPT)联合人乳头瘤病毒(HPV)-DNA检测诊断宫颈病变的临床意义。方法对我院妇产科门诊2010年1月至9月就诊的444例妇女实施宫颈LPT及HPV-DNA检测。对不典型鳞状上皮细胞(ASCUS)以上者和(或)HPV-DNA阳性者进行宫颈组织活检,活检结果高于轻度宫颈上皮内瘤变(CINⅠ)的为阳性。结果 444名LPT检查者,ASCUS以上者50例,HPV-DNA阳性194例,其阳性检出率分别为11.3%、43.7%,LPT及HPV-DNA均为阳性者31例,检出率为72.8%,其中CINⅠ~Ⅲ23例,宫颈癌5例,诊断符合率为93%。病理结果示:CINⅠ18例,CINⅡ32例,CINⅢ34例,宫颈癌12例,HPV感染率分别为44%(8/18)、28%(9/32)、56%(19/34)、75%(9/12)。结论癌前病变与HPV感染密切相关,LPT结合HPV-DNA检测可提高宫颈病变诊断率。

Abstract:Objective To investigate the significance of LPT combined with HPV-DNA detection in the screening of cervical pathology changes.Methods To take LPT combined with HPV-DNA detection for the 444 clinical visitors of our hospital during January to September in 2010,and cervix histology biopsy was performed for those whose result is above ASCUS or HPV-DNA(+).The positive rate are higher than cervical intraepithelial neoplasia(CINⅠ).Results In the 444 cases who took LPT,50 were above ASCUS,194 were HPV-DNA(+),the positive rate are 11.3% and 43.7% respectively.Both LPT(+) and HPV-DNA(+) have 31(72.8%)cases,among which CINⅠ~Ⅲ 23,cervical cancer 5.The coincidence rate is 93%.The pathology consequence shows: 44%(8/18),28%(9/32),56%(19/34) and 75%(9/12),respectively.Conclusion Cervical lesion is closely related with HPV.LPT combined with HPV-DNA detection can increase the prediction rate of cervical lesion.
