闭合性胰腺损伤的诊治体会

目的 探讨提高闭合性胰腺损伤的早期诊断率和选择合理的手术方法.方法 对26例闭合性胰腺损伤患者的诊断,手术方法,手术效果及并发症的处理进行总结分析.结果 26例中,死亡1例.17例修补加引流术后,发生胰瘘6倒,经保守治疗而愈;2例形成假性胰腺囊肿,二期手术囊内引流而愈.结论 胰腺损伤术前很难做出诊断.诊断的关键是剖腹探查;胰腺损伤的手术方式较多,应根据胰腺损伤的具体情况选择合理的手术方式,才能减少并发症和死亡率的发生.

Early Diagnosis and Treatment of Closed Pancreatic Injury

Objective TO explore the early diagnosis and operative treatment of closed pancreatic injuries. Methods The diagnosis, surgical treatment, operative effects and operative complications of 24 cases of pancreatic injury were retrospectively analyzed, Rusults In the 26 cases, operative complications occurred and 1 died. Among 17 cases with repairing plus drainage ,6 cases occured pan- creatic fistula, cured by conservative treatment. 2 cases occured pseudopancreatic cystoma. Conclusion Pancreatic injury can hardly be diagnosed preoperatively. Exploratory laparatomy is the key to its diagnosis. Different operative procedures can be used and appropriate methods should be chosen according to the injury degree so as to recluce the operative complication s and mortality.
