抗环瓜氨酸多肽抗体的临床意义及干扰因素分析

目的研究抗环瓜氨酸多肽(CCP)抗体对于早期类风湿关节炎(RA)的诊断价值,并探讨应用ELISA法测定抗CCP抗体时不同临床标本对于实验结果的影响。方法52例诊断为RA的患者和其他结缔组织病患者60例用ELISA方法检测抗CCP抗体,同时用间接免疫荧光法检测抗角质蛋白抗体(AKA),并用溶血、高血脂、黄疸标本做干扰实验。结果抗CCP抗体对RA诊断的敏感性为59.62％,特异性为96.67％;干扰实验中溶血、高血脂、高浓度黄疸对于检测结果无显著影响(P>0.05);中度黄疸使测定结果下降,有显著性差异(P<0.05)。结论抗CCP抗体对于早期RA有极高的诊断价值,联合检测AKA效果更好;ELISA方法检测抗CCP抗体是一种准确灵敏的方法。

The clinical significance of anti-cyclic critrullinated polypeptide antibody and the analysis of interference factors

Objective To study the diagnostic value of anti-cyclic critrullinated polypeptide (CCP) antibody (AKA) for early rheumatoid arthritis and to investigate the influences of different samples on experimental result. Methods The anti-CCP antibody in 52 patients with RA and 60 patients with other CTD was detected by ELBA. The anti-keratin antibody (AKA) in these patients was detected by indirect immunofluorescent test. The interference test was done with the samples of hemolysis, hyperlipidemia and icterus. Results The sensitivity of anti-CCP antibody was 59.62% and the specificity was 96.67% ;the samples of hemolysis, hyperlipidemia and high concentration in icterus had no significant influence on the experimental result ( P > 0.05), however, middle concentration of icterus made anti-CCP antibody significantly lower ( P < 0. 05). Conclusion Anti-CCP antibody has very important diagnostic value for early RA, and the result will be better if that is combined with detection of AKA, which is a sensitive and exact way of detecting anit-CCP antibody by ELISA.
