踝臂指数评价2型糖尿病合并外周动脉疾病的价值

目的观察2型糖尿病患者合并外周动脉疾病（PAD）的发病情况，探讨踝臂指数在临床上的应用价值。方法选取在住院的T2DM患者320例，收集相关临床资料并检测相关生化指标。采用多普勒血流探测仪专人测定糖尿病患者的足背动脉、胫后动脉与肱动脉的比值（踝臂指数），使用SPSS13．0统计软件进行数据分析和处理。结果2型糖尿病合并PAD的患者为92例，总的发病率为28．75％。2型糖尿病合并PAD组患者的血压、糖化血红蛋白（HbMc）、总胆固醇（TC）、甘油三酯（TG）、低密度脂蛋白（LDL）、餐后2hC肽（P2h—CP）、血尿酸（UA）明显高于与2型糖尿病非PAD组（P〈0．05）。PAD与年龄、血压、血尿酸有相关性（P〈0．05）。结论增龄、高血糖、高血压、高低密度脂蛋白、高总胆固醇、高甘油三酯、高C肽水平及高尿酸是下肢动脉病变的危险因素；踝臂指数测定可作为早期诊断PAD的首选和常规检查手段。

Evaluation of ankle brachial index on type 2 diabetes patients with peripheral artery disease

Objective To observe the incidence of the peripheral artery disease(PAD) in type 2 diabetes patients and to explore the clinical value of ankle brachial index.Methods A total of 320 cases were recruited from the department of endocrinology in our study.Collected clinical data to measure biochemical indicators.The data were analyzed by SPSS13.0 software.Results The results revealed that there were 92 type 2 diabetes patients with PAD.The overall disease incidence is 28.75%.Blood pressure,HbAlc,total cholesterol(TC),triglycerides(TG),low-density lipoprotein(LDL),postprandial two hours of C-peptide(P2h-CP) and serum uric acid(UA) in type 2 diabetes with PAD group are significantly higher than the non-PAD group(P<0.05).They are relevant with age,blood pressure,and blood uric acid(P<0.05).Conclusion Age,high blood glouse,high blood pressure,high LDL,high total cholesterol,high triglycerides,high C-peptide levels and high uric acid are risk factors of peripheral arterial disease.Ankle-brachial index is the first choice for the early diagnosis of PAD and conventional means of inspection.
