老年患者医院下呼吸道感染铜绿假单胞菌耐药性分析

摘    要:目的 观察老年患者医院下呼吸道感染铜绿假单胞菌耐药特点,为临床合理选用抗菌药物和预防泛耐药菌的发生提供依据.方法 回顾性分析2006年9月-2011年6月624例老年患者,医院下呼吸道感染铜绿假单胞菌药敏试验结果.结果 铜绿假单胞菌对抗菌药物的耐药较严重,耐药率以妥布霉素15.5%最低,其次是头孢哌酮/舒巴坦为22.1%;而磺胺甲噁唑/甲氧苄啶耐药率最高,达65.8%,其次为左氧氟沙星为65.4%、头孢噻肟为62.3%;对亚胺培南耐药率已达39.6%;泛耐药菌的检出率为5.6%.结论 老年患者医院下呼吸道感染铜绿假单胞菌的耐药性严重,应根据药敏试验结果合理选择抗菌药物,防止泛耐药菌的产生.

Abstract:OBJECTIVE To observe the characteristics of the drug resistance of Pseudomonas aeruginosa causing lower respiratory tract infections in senile patients so as to provide bases for clinically reasonable choice of antibiotics and the prevention of pandrug-resistance.METHODS The results of drug susceptibility testings for P.aeruginosa causing lower respiratory tract infections in 624 senile patients from Sep 2006 to Jun 2011 were analyzed retrospectively.RESULTS The drug resistance of P.aeruginosa was very serious,the drug resistance rate to tobramycin was the lowest(15.5%),followed by cefoperazone/sulbactam(22.1%);the resistance rate to sulfamethoxazole/trimethoprim was the highest(65.8%),followed by levofloxacin(65.4%) and cefotaxime(62.3%);the resistance rate to imipenem has reached 39.6%;the detection rate of pandurg-resistant bacteria was 5.6%.CONCLUSION The drug resistance of P.aeruginosa causing lower respiratory tract infection in senile patients is so serious that it is necessary to reasonably choose antibiotics on the basis of drug susceptibility testing so as to prevent the pandrug-resistant bacteria.
