肛肠疾病外科手术后抗菌药物应用合理性分析

目的 分析肛肠疾病外科术后抗菌药应用状况及合理性.方法 回顾性分析2009年1月-2011年10月入医院肛肠科治疗的297例患者临床资料,对各类抗菌药物的品种、所占比例、给药途径、排序等进行统计分析.结果 术后使用的各类抗菌药物中,头孢菌素类占44.4％、青霉素类占17.8％、喹诺酮类占11.1％、大环内酯类占8.4％、β-内酰胺酶类占9.8％是最常用的抗菌药物；术后有73.7％的患者使用了≥2种抗菌药物联合用药,其中二联占45.8％,有效率为94.8％,三联占18.2％,有效率为90.7％,≥四联占9.7％,有效率为86.2％,但抗菌药物使用方式及预后与单用结果差异无统计学意义.结论 应当进一步加强抗菌药使用管理和监督,强化外科临床医师抗菌药物合理使用的意识.

Rationality of use of antibiotics after anorectal disease surgery

OBJECTIVE To analyze the current status of application of antibiotics as well as its rationality after anorectal diseases surgery.METHODS The clinical data of 297 patients who enrolled in the anorectal department from Jan 2009 to Oct 2011 were analyzed retrospectively.The varieties of the antibiotics,proportions,delivery routes and orders were taken for statistical analysis.RESULTS Of all the varieties of antibiotics used after the surgery,cephalosporins accounted for 44.4%,penicillins 17.8%,quinolones 11.1%,macrolides 8.4%,and beta-lactamases 9.8%,which were the most commonly used antimicrobial drugs;there were 73.7% of patients who took the combination use of more than two antibiotics,among which two-drug use accounted for 94.8% with the effective rate of 94.8%,three-drug use accounted for 18.2% with the effective rate of 90.7%,and more than four-drug use accounted for 9.7% with the effective rate of 86.2%,but there was no statistical difference between the use of antibiotics and prognosis and the single-drug use.CONCLUSION It is necessary to further intensify the administration and supervision of the antibiotics as well as enhance the surgeons’ awareness about reasonable use of antibiotics.
