颅脑手术医院感染的临床分析

目的 分析神经外科颅脑手术后医院感染的发生情况及其相关危险因素,探讨预防及控制对策.方法 对2010年3-9月神经外科接受颅脑手术的住院患者发生医院感染的部位、病原菌种类、相关危险因素等进行调查.结果 共调查465例颅脑手术住院患者,发生医院感染56例,感染率为12.03％,感染部位以下呼吸道及颅内感染居多,分别占46.15％、43.07％；logistic回归分析显示,感染的主要危险因素为手术时间、ICU住院天数及术前住院天数.结论 神经外科颅脑手术医院感染率较高,应重视颅脑手术医院感染的预防与控制,针对不同的危险因素采取相应的预防措施.

Clinical analysis of nosocomial infections due to intracranial surgery

OBJECTIVE To analyze the distribution status and relevant risk factors of nosocomial infections due to intracranial surgery in neurosurgery department and to explore the prevention and control countermeasures.METHODS The sites,reasons,and bacterial culturing of hospital infections in hospitalized patients of neurosurgery department with intracranial surgeries from Mar 2010 to Sep 2010 were investigated and analyzed.RESULTS Of totally 465 hospitalized patients undergoing intracranial surgeries investigated,nosocomial infections occurred in 56 patients with the infection rate of 12.03%;the lower respiratory tract infections and intracranial infections ranked the first and second place(46.15%,43.07%);the logistic regression analysis indicated that the major risk factors relevant to nosocomial infections were the operation duration,ICU stay and the hospital stay before surgery.CONCLUSION The incidence of nosocomial infections in neurosurgery department with intracranial surgeries is comparatively high,especially on the patients suffering from encephalic tumor so we should strengthen the prophylaxis and surveillance of nosocomial infections in neurosurgery department with intracranial surgeries,taking relevant preventive countermeasures to reduce postoperative infections by the type of different risk factors.
