摘    要:目的 神经外科重症监护病房(NSICU)和神经外科普通病房患者感染鲍氏不动杆菌(ABA)的耐药率,为临床合理用药提供依据.方法 以回顾性分析的方法,对2010年1月-2011年1月两个病房患者感染ABA的部位及耐药率进行比较分析.结果 NSICU和普通病房分别检出ABA 83株和22株,感染部位均以下呼吸道感染为主;除阿米卡星、妥布霉素、多黏菌素B外,NSICU检出的ABA对其他抗菌药物的耐药率,为74.7%~100.0%,而普通病房检出的ABA除对氨曲南和四环素的耐药率较高外,对其他抗菌药物具有很好的敏感性;两个病房相比,除阿米卡星、妥布霉素、多黏菌素B外,NSICU检出的ABA对其他抗菌药物的耐药率均明显高于普通病房,差异有统计学意义(P<0.05).结论 NSICU感染的ABA呈多药耐药性,临床应合理使用抗菌药物,以提高疗效和减缓耐药菌株的产生.

Abstract:OBJECTIVE To compare the drug resistance rates of Acinetobacter baumannii causing infections in the patients in NSICU and general wards of department of neurosurgery so as to provide bases for reasonable use of antibiotics.METHODS By the means of retrospective analysis,the sites of the patients with A.baumannii infection and the drug resistance rates from Jan 2010 to Jan 2011 were compared and analyzed.RESULTS There were 83 strains and 22 strains of A.baumannii which were respectively isolated from NSICU and the common wards,the lower respiratory tract was the main site with infections;the resistance rates of A.baumannii isolated from NSICU were relatively high to all the antibiotics except amikacin,tobramycin,and polymyxin B,varying from 74.7% to 100.0%,however,the drug resistance rates of A.baumannii isolated from general wards were relatively high to aztreonam and tetracycline,in addition,A.baumannii was susceptible to other antibiotics;as compared with the two wards,the drug resistance rates of A.baumannii isolated from NSICU were significantly higher than those from the common wards except the resistance rates to amikacin,tobramycin,and polymyxin B,the differences were statistically significant(P<0.05).CONCLUSION A.baumannii causing infections in NICU shows multidrug-resistance,it is necessary to reasonably use antibiotics so as to improve the clinical efficacy and delay the outbreak of resistant strains.
