摘    要:目的 研究抗菌药物专项整治对抗菌药物使用强度及费用的作用效果.方法 采用横断面研究的设计,通过医院信息系统导出该院2010年1月1日-2011年12月31日所有出院患者静脉注射使用抗菌药物数据,比较整治前、后抗菌药物使用强度及费用.结果 专项整治后,抗菌药物使用强度从89.05 DDDs下降至69.04DDDs(P=0.043),重点管理的抗菌药物包括第三、四代头孢菌素、喹诺酮类以及碳青霉烯类等,使用强度均显著下降,下降率分别为42.26%、47,31%、28.66%、30.85%,抗菌药物人均使用费用由1224.61元下降至1031.24元,住院日均使用费用由89.55元下降至76.21元,但全院抗菌药物使用强度仍然>40 DDDs.结论 抗菌药物专项整治可显著降低抗菌药物的消耗量和费用,优化抗菌药物使用种类.

Abstract:OBJECTIVE To evaluate the effect of strict antibiotic restriction policy(SARP) on the use intensity and the cost of antibiotics.METHODS We performed a cross-sectional study designed to evaluate the intensity and cost of antimicrobial agents before and after the rectification,and the hospital information system was used to extract all eligible data of discharged patients with intravenous use of antibiotics from Jan 1,2010 to Dec 31,2011.RESULTS After the special rectification,the use intensity of antibiotics decreased from 89.05 DDDs to 69.04 DDDs(P=0.043);the key antibiotics involved in the management included the third and fourth generation cephalosporins,quinolones and carbapenems and others,the use intensity of which was all significantly dec eased,decreasing respectively by 42.26%,47.31%,28.66%,and 30.85%,the average cost of each patient dropped from 1224.61 yuan to 1031.24 yuan,and the antibiotic charges per patient-day dropped from 89.55 yuan to 76.21 yuan,however,the use intensity of the whole hospital remained higher than 40DDDs.CONCLUSION SARP can significantly reduce the consumption and cost of the antibiotics as well as optimize the types of using antibiotics.
