小儿肺炎支原体感染后免疫功能变化的研究

摘    要:目的 探讨小儿肺炎支原体感染后免疫功能的变化规律及其临床意义.方法 选取2009年7月-2011年11月于医院进行治疗的65例肺炎支原体感染患儿为观察组,同时选取同期的65名健康体检小儿为对照组,将两组小儿的血清免疫球蛋白、红细胞免疫及外周血T淋巴细胞亚群的水平进行检测及比较.结果 观察组的IgA(1.22±0.16)g/L、IgM(0.91±0.18)g/L、IgG(9.20±2.12)g/L、补体C3(0.95±0.22)g/L、C4(0.24±0.04)g/L及CD3+ (64.26±5.06)%、CD4+(42.54±4.21)%、CD4/CD8(1.30±0.19)、红细胞免疫复合物花环率(5.12±1.02)%、C3b受体花环率(10.24±1.19)%、免疫黏附促进因子(53.89±5.11)%均低于对照组,CD8+(32.31±3.18)%、红细胞免疫抑制因子(30.28±2.27)%高于对照组,且急性期差异幅度大于恢复期,差异均有统计学意义(P<0.05).结论 小儿肺炎支原体感染后免疫功能处于相对低下的状态,调节其免疫状态对于改善疾病状态发挥积极作用.

Abstract:OBJECTIVE To study the clinical significance and variation of immune function of the children with Mycoplasma pneumoniae infection.METHODS A total of 65 children with Mycoplasma pneumoniae infection who enrolled hospital from Jul 2009 to Nov 2011 were selected as the observation group,and 65 healthy children at the same time were selected as the control group,then the serum immunoglobulin,erythrocyte immune function and peripheral blood T lymphocyte subsets of two groups were detected and compared.RESULTS The IgA(1.22±0.16)g/L,IgM(0.91±0.18)g/L,IgG(9.20±2.12)g/L,complement C3(0.95±0.22)g/L and C4(0.24±0.04)g/L,CD3+(64.26±5.06)%,CD4+(42.54±4.21)%,CD4/CD8(1.30±0.19),Red cell immune complex rosette rate(5.12±1.02)%,red cell C3b receptor rosette rate(10.24±1.19)% and red cell immune adherence promoting factor(30.28±2.27)% of the observation group were all lower than those of the control group,while both CD8+(32.31±3.18)% and red cell immune adherence inhibition factor(53.89±5.11)% were higher than those of the control group,there was larger difference range in the acute stage than in the convalescence stage,the differences were statistically significant(all P<0.05).CONCLUSION The immune function of the children with Mycoplasma pneumoniae infection is relatively low,and the regulation of immune function plays an active role in improving the outcome of disease.
