60例狼疮性肾炎的临床与病理

目的：探讨狼疮性肾炎（LN）患者的临床表现、实验室检查与病理类型之间的关系。方法：住院LN患者60例在B超下行肾活检及常规实验检查并进行病理分型。结果：60例LN病人中病理类型以Ⅳ型最多,占58.3%,其次是Ⅴ型（35.0%）,Ⅳ型多表现为肾病综合征,Ⅴ型多表现为肾炎综合征及肾病综合征,激素和免疫抑制剂是治疗LN的主要药物。结论：系统性红斑狼疮的临床表现复杂,肾脏是主要受累器官。LN的病理类型与临床表现有一定关系,肾活检对判断疾病活动性、指导治疗与估计预后有重要意义。

Clinical Data and Pathology of 60 Cases of Lupus Nephritis

Objective:To explore the clinical manifestation of patients with lupus nephritis(LN),and the relationship between laboratory examination results and pathologic classification.Methods:Renal biopsy and routine laboratory tests were performed to 60 hospitalized LN patients.The pathologic classification of LN was made according to the World Health Organization standards(1995).Results:In 60 cases,the most common type was type Ⅳ(58.3%),and Type Ⅴ ranked the second(35.0%).Patients with type Ⅳ mainly demonstrated as nephritic syndrome(NS),and patients with type Ⅴ mainly demonstrated as glomerulonenephritis and nephritic syndrome.The main drugs for treatment of LN were hormone and immune inhibitor.Conclusions:Clinical manifestation of systemic LN is complex,and kidney is the mainly involved organ.Pathologic classification correlates with clinical manifestation to some degree.Renal biopsy is significant in judging disease activity,guiding treatment,and estimating prognosis.
