胆管细胞型肝癌的CT及DSA表现

目的探讨胆管细胞型肝癌的CT表现及DSA征象,提高胆管细胞型肝癌的诊断水平。方法19例经病理证实的胆管细胞型肝癌的常规CT平扫及双期强化扫描,其中6例同时行延迟扫描,15例行DSA检查。结果CT主要表现:平扫均表现为单发低密度病灶,边界不清;动脉期:所有病灶边缘呈不规则环状轻、中度强化;门脉期:病灶进一步强化呈网格状;延迟期:病灶不均匀向心性增强,略高于正常肝实质。DSA表现:供血动脉轻、中度增粗、僵直,肿瘤染色呈云片状、羽毛状,导管越是超选,肿瘤染色越明显。结论胆管细胞型肝癌的CT及DSA表现具有特征性,对诊断及鉴别诊断具有重要价值。

CT and DSA Findings of Intrahepatic Cholangiocarinoma

Objective To evaluate CT and DSA features of liver cholangiocarcinoma,in order to improve the knowledge in diagnosing liver cholangiocarcinoma.Methods CT plain scan and contrast-enhanced CT scan were performed in 19 patients with liver cholangiocarcinoma confirmed by pathology, of them,delay contrast-enhanced scans were performed in 6 cases.DSA were performed in 15 patients.Results CT findings of lesions were:solitary low dense with obscure border on plain CT scan;lower dense with irregular rings enhancement in mild or middle degree on hepatic artery phase;by reticular enhancement on portal vein phase;heterogeneous with the tendency of concentric enhancement on delay phase and the dense was light higher than that of normal liver parenchema.DSA findings:the feeding arteriae were augment in mild to middle degree and became rigid,the lesions manifested by pathch and feathery-like tumor stain,the more superselective the catheter inserted,the more obvious the tumor stain was.Conclusion Findings of CT and DSA of liver cholangiocarcinoma are of some features,which are of important value in diagnosis and differential diagnosis of liver cholangiocarcinoma.
