彩色多普勒超声在新生血管性青光眼血流动力学改变中的应用价值

摘    要:目的 应用彩色多普勒超声探讨新生血管性青光眼(NVG)主要动脉的血流动力学变化和规律.方法 NVG 患者45例(NVG组),共45只眼(单眼患病),正常成人28例(对照组),56只眼.观察两组眼压和视野的变化,使用彩色多普勒超声检测开角型与闭角型NVG患者早期和中晚期的眼动脉、视网膜中央动脉(CRA)和睫状后短动脉(SPCA)的峰值收缩期血流速度(PSV)、舒张末期血流速度(EDV)及阻力指数(RI)的变化,结果与对照组对比分析.结果 与对照组比较,NVG 患者早期各主要动脉的PSV、EDV及RI值差异无统计学意义;中晚期时NVG组眼动脉和SPCA的PSV与RI值显著高于对照组,差异有统计学意义(P 〈 0.01),SPCA的EDV则显著低于对照组,差异有统计学意义(P 〈 0.01).开角型NVG患者中CRA的PSV和EDV与对照组比较,差异无统计学意义,仅RI值显著升高,差异有统计学意义(P 〈 0.05).结论 眼动脉和SPCA的PSV和RI值的显著增高与分型无关,RI值是判定患眼局部循环较为可靠的指标.

Abstract:Objective To investigate the hemodynamic changes of ophthalmic artery in neovascular glaucoma by color Doppler ultrasound. Methods Forty-five neovascular glaucoma(NVG) patients with 45 eyes and 28 normal patients with 56 eyes were enrolled in this study. The intraocular tension and visual field were determined simultaneously. The changes of peak systolic velocity (PSV), end diastolic velocity (EDV) and resistant index ( RI ) of the ophthalmic artery ( OA ), central retinal artery (CRA) and the short posterior ciliary artery (SPCA) were measured by color Doppler ultrasound. The results were compared with those of control group. Results Compared with control group, there was no significant difference of PSV, EDV and RI of each artery in patients with early NVG, while at the middle and late phase of NVG, the PSV and RI values of OA and SPCA were increased significantly (P 〈 0.01 ), and EDV value of SPCA was decreased significantly, there was significant difference(P〈 0.01 ). Compared with control group, only RI value of CRA was increased in the open angle type of NVG(P 〈 0.05 ), there was no significant difference of PSV and EDV values of CRA. Conclusion The increased RI and PSV of OA and SPCA have no correaltion with typing, the value of RI is a reliable parameter in evaluating local mierocirculation of the suffered eyes.
