大剂量尿激酶溶栓治疗急性脑梗塞的临床研究

目的 探讨早期应用大剂量尿激酶溶栓治疗脑梗塞的临床疗效。方法 将80例急性脑梗塞病人分为溶栓组、对照组。溶栓组应用生理盐水200ml加尿激酶150万U，在20～30min内滴完，溶栓12h后用低分子肝素7500U皮下注射，1次／24h，共5～7d。对照组1次／d皮下注射低分子肝素0．4ml，连用7d。溶栓前、溶栓后24h及溶栓后14d进行神经功能缺损评分、疗效评定及治疗前后血流变检测。结果 溶栓组疗效明显优于对照组。结论 只要严格掌握溶栓治疗的时间窗、适应症及禁忌症，应用大剂量尿激酶在急性脑梗塞的超早期进行静脉溶栓治疗是安全的、有效的。

Clinical research on treating acute cerbral infarction with large dose of urokinase

Objective To evaluate the therapeutic effects of treating acute cerbral infarction with large dose urokinase at the early stage. Methods 80 patients with acute cerbral infarction were divided into thrombolysis group and control group . 150wu of urokinase in 200ml normal saline were administered via intravenous drip infusion within 20 - 30rain, 7500U of low molecular heparin were administered via hypodermic injection 12h after thrombolysis per 24h , totally 5 to 7 days. 0.4ml of low molecular heparin were administered via hypodermic injection for consecutive 7 days in control group. Neurelogic impairment score , therapy evaluation and blood rbeology index test pro and post treatment were evaluated before thrombolysis, 24h after thrombolysis and 14 days after thrombolysis were compared and the values of them were compared . Results The tberapeutic effect of thrombolysis group was significantly better than that of control group. Conclusion treating acute cerbral infarction with large dose of urokinase in the early stage is safe and effective if window time ,indication and contra-indication is strictly controlled .
