肺癌患者69例血清肿瘤标记物测定分析

目的探讨肿瘤标记物在肺癌诊断中的意义。方法收集各型肺癌患者的血清标本69例，正常对照组30例，对这些血清标本进行CEA、CA125、CA199、CA242、NSE五项指标的测定。结果肺癌各组的CEA、CA125均值均高于正常参考值，肺腺癌组与正常组比较有显著差异。正常组及肺癌各组CA199 CA242的检测值均在正常范围内。正常组及肺腺癌、肺鳞癌的NSE在正常参考值内，只有小细胞肺癌的NSE明显高于正常参考值，与正常组比较，有显著差异。各项标记物的特异度均较高，但灵敏度不同。肺腺癌的CEA、CA125、CA242的灵敏度较高，小细胞肺癌的CEA、CA125、NSE的灵敏度较高。选择二项阳性作为诊断指标，则肺癌、肺腺癌、小细胞肺癌的阳性率均有50％以上，与正常组比较均有显著差异。结论CEA、CA125、CA242可作为肺腺癌；NSE、CA125、CEA可作为小细胞肺癌较好的肿瘤标记物。联合检测中二项阳性率对肺癌诊断最有价值。

Lung cancers 69 cases sufferer's serum tumor marker measurement analysis

Objective To study the significance of the explicit tumor marker in the diagnosis of lung cancer .Methods The serum samples of 69 lung cancer patients was collected and 30 serum samples from comparison control group. Five measurements which were CEA, CA125, CA199, CA242, NSE was carried out to these serums samples. Results The average value of CEA, CA125 of every group was higher than the normal reference . It also showed that the adenocarcinoma group was obviously different from normal group. The average value of CA199, CA242 measurements of normal group and all lung cancer groups were within normal range. The NSE value of the normal group, the adenocarcinoma group and the squamous cell carcinoma failed in normal reference range, but the NSE value of the small cell lung cancer was obvious higher than the normal reference value which showed the difference from the normal group. The specificity of various tumor marker wee quite high, but the sensitivity was different. The sensitivity of the CEA, CA125, CA242 of the lung gland cancer is higher, the sensitivity of the CEA, CA125, NSE of small cell lung cancer is higher. So, the positive ratio of lung cancer, adenocarcinoma and small cell lung cancer could be more than 50% if the results of the two measurements from above 5 measurements is positive because of the difference from the normal. Conclusion CEA,CA125 and CA242 could be the tumor marker of lung adenocarcinoma. CEA, CA125 and NSE could be the tumor marker of small cell lung cancer. It is most valuable to the diagnosis of lung cancer if two experiments from the five experiments show positive.
