妊娠高血压疾病孕妇眼底病变与眼动脉血流动力学特点的相关性研究

目的:观察彩超检测孕妇眼动脉血流参数与妊娠高血压疾病(HDCP)孕妇眼底病变的相关性,探讨其在临床HDCP诊断和治疗中的价值。方法:以40例正常孕妇作为对照组,测定73例HDCP组孕妇眼动脉搏动指数(PI)、阻力指数(RI)及收缩期第二峰(B)与第一峰(A)的比值(B/A);同时检查两组眼底改变,比较不同程度眼底病变时眼动脉血流参数的变化。结果:①眼动脉的PI、RI值与孕妇年龄及孕龄无明显相关(P>0.05),而B/A值与孕妇年龄有显著相关性(P<0.001);②HDCP孕妇眼动脉的PI、RI值明显低于正常孕妇(P<0.05),而B/A值则明显升高(P<0.05);③HDCP孕妇眼底病变程度与PI、RI及B/A值显著相关(P<0.001),眼底病变越严重,PI、RI值越低,而B/A值越高:④眼底改变程度与临床病情严重程度显著相关(P<0.001)。结论:检测孕妇眼动脉的血流参数结合其眼底病变情况对HDCP的临床诊断及治疗具有明显的参考意义。

The relationship between the pathological changes of eyegrounds and ophthalmic artery flow parameters in hypertensive disorder complicating pregnancy patients

Objective:To investigate the relationship between the pathological changes of eyegrounds and ophthalmic artery(OA) flow parameters measured by Doppler ultrasonography in hypertensive disorder complicating pregnancy(HDCP) patients.Methods:The OA flow parameters,including pulsatility index (PI),resistent index(RD)and ratio of systolic second peak velocity(B) to systolic first peak velocity(A)-B/A,were measured by Doppler ultrasonography for 73 HDCP patients.Meanwhile,the pathological changes of eyegrounds in all patients were examined in parallel 40 normal pregnant women were settled as the control group.Results: ①PI,RI did not correlate with age(P>0.05) and gestational age(P>0.05) but B/A correlated with age(P<0.001) markedly;②PI,RI decreased and B/A increased significantly in HDCP patients than that in control group(P<0.01);③PI,RI and B/A were highly correlated with pathological changes of eyegrounds in HDCP patients(P<0.001).The worse the patients pathological changes of eyegrounds,the lower PI,RI,but the higher B/A(P<0.01);④The HDCP patient's eyegrounds pathological changes were highly correlated with the patient's condition(P<0.001).Conclusion:The ophthalmic artery(OA) flow parameters binding with the pathological changes of eyegrounds are valuable for the clinical diagnosis for HDCP patients.
