肝功能检测与妊娠期肝内胆汁淤积症的相关性分析

目的 分析肝功能检测与妊娠期肝内胆汁淤积症之间的关系.方法 收集2012年4月至2013年4月我院收治的64例妊娠期肝内胆汁淤积症患者的临床资料,根据患者肝功能情况随机将患者分为转氨酶正常组和转氨酶异常组,各32例.对比两组患者肝功能与血清总蛋白代谢异常,分析母儿并发症与妊娠结局的关系.结果 转氨酶正常组产妇失眠、头昏、乏力等症状发生率为12.50％,明显低于转氨酶异常组的28.13％,差异有统计学意义（P＜0.05）.转氨酶正常组出现胎膜早破、胎儿窘迫、死产、早产、羊水异常、新生儿窒息、新生儿黄疸等并发症发生率为34.38％,明显低于转氨酶异常组的56.25％,差异有统计学意义（P＜0.05）.两组患者TB值、DB值对比,差异无统计学意义（P＞0.05）.转氨酶正常组ALT值、TP值、A值、G值、HBsAg阳性等指标明显优于转氨酶异常组,差异有统计学意义（P＜0.05）.转氨酶正常组患者ALT例数、A例数、G例数、A/G倒置率明显低于转氨酶异常组,差异有统计学意义（P＜0.05）.结论 肝功能检测与妊娠期肝内胆汁淤积症呈正相关.肝功能检测可以及时反映ICP中期妊娠肝功能及血清总蛋白的异常变化,有利于降低母儿并发症发生率,不断改善妊娠结局,值得临床推广.

Correlation between liver function test and intrahepatic cholestasis during pregnancy

Objective To analyze the correlation between liver function test and intrahepatic cholestasis during pregnancy.Methods The clinical data of 64 patients with intrahepatic cholestasis during pregnancy admitted into our hospital from April,2012 to April,2013 were collected.According to the patients＆#39; liver function,the patients were randomly divided into a normal transaminase group and an abnormal transaminase group,32 for each group.The liver function group and serum total protein metabolic abnormality were compared between the 2 groups.The relationship between maternal and infant complications and pregnancy outcomes was analyzed.Results The incidence of complications,such as insomnia,dizziness,fatigue,etc.,was 12.50% in the normal transaminase group and was 28.13% in the abnormal aminotransferase group,with a statistical difference （P 〈 0.05）.The incidence of premature rupture of membranes,fetal distress,stillbirth,premature birth,abnormal amniotic fluid and neonatal asphyxia,neonatal jaundice,etc.was 34.38%in the normal transaminase group and was 56.25% in the abnormal transaminase group,with a statistical difference （P 〈 0.05）.There were no statistical differences in TB and DB values between the 2 groups（P 〉 0.05）.The values of ALT,TP,A,G,HBsAg positive,etc.were better in the normal transaminase group than in the abnormal transaminase group,with statistical differences （P 〈 0.05）.Fewer patients had abnormal ALT,A and G and the A/G inversion rate was lower in the normal transaminase group than in the abnormal transaminase group,with statistical differences （P 〈 0.05）.Conclusions Liver function test positively correlates with intrahepatic cholestasis.Taking liver function test can timely reflect the ICP second-trimester liver function and abnormal changes of serum total protein,lower incidence of maternal and infant complications,and continuously improve the pregnancy outcomes and is worth being clinically generalized.
