应用反取皮植皮联合带蒂胸脐皮瓣治疗前臂脱套伤

目的 报道当前臂及手腕部脱套伤时,应用反取皮植皮联合带蒂胸脐皮瓣治疗的临床效果.方法 2007年9月-2011年11月,对12例前臂及手腕部脱套伤,利用反取皮植皮联合带蒂胸脐皮瓣修复,以达到保全肢体外形及功能,覆盖创面的目的.结果 12例植皮及皮瓣全部成活,术后随访10例,随访时间5个月～2年,外形及功能满意,质地良好,无色素沉着,其中3例皮瓣明显臃肿,需二次皮瓣修薄整形.供区无明显并发症.结论 前臂及手腕部脱套伤,利用反取皮植皮联合带蒂胸脐皮瓣修复,手术方法简单,安全可靠,不需吻合血管,能达到精细修复的目的,明显提高功能.

Application of anti skin grafting combined with pedicled thoracoumbilicus flap in the treatment of forearm degloving injury

Objective To report the clinical effects of degloving injury of the forearm and wrist region by application of anti skin grafting combined with pedicled thoracoumbilicus flap. Methods From September 2007 to November 2011, 12 cases with degloving injury of the forearm and wrist region were repaired with anti skin grafting combined with pedicled thoracoumbilicus flap, in order to achieve the preservation of the limbs appearance and function, to cover the wound for the purpose. Results 20 flaps completely survived, the appearance and functional results were satisfactory with following up for 5 months to 2 years. The color and texture of the flaps were good. There was no postoperative complication in the donated place. There was no pigmentation in the recipient site. 3 flaps was obvious fat and need be thined. Conclusion Degloving injury of the forearm and wrist region were repaired with anti skin grafting combined with pedicled thoracoumbilicus flap, the operation is simple, safe and reliable, and does not need vascular anastomosis, can achieve the fine repair, obvious greatly improve the function.
